| 目的 探讨骨水泥强化椎弓根钉钉道治疗伴骨质疏松胸腰椎爆裂骨折的临床疗效。方法 回顾分析2007年8月～2011年2月收治的11例伴骨质疏松胸腰椎爆裂骨折患者的临床资料，平均56.1岁，无重大基础病存在，经脊柱后入路椎弓根钉撑开复位，钉道采用骨水泥灌注加强。术后随访7个月～42个月，平均27月。结果 均耐受手术，平均手术时间为92.7±5.3min，平均失血量为270±6.1ml，术后腰背痛明显改善，椎体高度恢复至90%左右，随访椎体高度角度丢失不明显，内固定无断裂、移位。结论 应用骨水泥强化椎弓根钉道治疗伴骨质疏松胸腰椎爆裂骨折，矫正及维持椎体复位效果好，完备的围手术期治疗、手术技巧的掌握和骨质疏松药物治疗是关键。 |

| Objective To evaluate the clinical efficacy of the treatment of thoracolumbar osteoporotic burst fractures with posterior pedicle screw fixation consolidated with PMMA. Methods Clinical data of 11 patients, who were diagnosed with thoracolumbar osteoporotic burst fractures from Aug 2007 to Feb 2011, were retrospectively analyzed. The patients had an average age of 56.1 years old and had no underlying fatal diseases. Vertebral canal reduction through posterior pedicle screw fixation consolidated with PMMA and cement injecting into screw hole were performed. All patients were followed up for 7-42 months (average 27 months) after the operation. Results All patients survived after the surgery. The average operating time was 92.7±5.3 minutes. The average bleeding volume was 270±6.1ml. Low back pain was significantly relieved after the operation. The height of fracture vertebral bodies recovered to 90% after the operation. The loss of height and angle was not significant during the follow-up period. No internal fixations had rupture or shifting. Conclusion The treatment of thoracolumbar osteoporotic burst fractures with posterior pedicle screw fixation consolidated with PMMA has a good effect on correcting and maintaining the reduction of vertebrae. Thorough perioperative treatment, practiced operation skills, and anti-osteoporosis medication are the key points to ensure good clinical effect. |
