目的 评估高露洁脱敏抛光膏对牙槽骨吸收、疏松患者龈上洁治术后牙本质敏感的临床治疗效果,并观察其对牙齿表面的光洁作用及对牙本质小管的封闭能力。方法 离体牙牙釉质洁治后,牙釉质脱敏抛光膏抛光后,劈开暴露牙本质后、牙本质脱敏抛光膏抛光后分别扫描电镜观察对牙齿表面的光洁作用及对牙本质小管的封闭能力。选择龈上洁治术后80名患者的160颗敏感牙,随机分为脱敏抛光膏组和非脱敏抛光膏组,脱敏抛光膏组采用脱敏抛光膏抛光,非脱敏抛光膏组采用无脱敏成分的抛光膏抛光。分别于抛光前、抛光后即刻,术后3d、1w、4w、8w,采用冷空气喷吹法检测患牙的敏感程度,记录视觉模拟疼痛评分(VAS值)。结果 扫描电镜观察显示脱敏抛光膏能抛光牙面的同时封闭大部分暴露开放的牙本质小管,但堵塞物较表浅。脱敏抛光膏组和非脱敏抛光膏组术后即刻、3d、1w、4w、8wVAS值有显著差异(P>0.05)。结论 使用高露洁脱敏抛光膏可有效缓解牙槽骨吸收、疏松患者龈上洁治术后牙本质敏感症状。 |

Objective To evaluate the clinical effect of Colgate in-office desensitizing paste on dentin hypersensitivity in patients with alveolar resorption and osteoporosis after ultrasonic scaling, and to observe its effect on tooth polishing and the occlusion of dentinal tubules. Methods The surface smoothness of teeth was observed under scanning electron microscope (SEM) after ultrasonic scaling and polishing in vitro. The occlusion of dentinal tubules on teeth was observed by SEM before and after the polishing using Colgate in-office desensitizing paste. A total of 160 teeth in 80 patients with the presence of dentin hypersensitivity after ultrasonic scaling were selected and randomly divided into 2 groups: in-office desensitizing paste group and polishing paste group. After ultrasonic scaling, teeth of patients in in-office desensitizing paste group were polished with Colgate in-office desensitizing paste, while teeth of patients in polishing paste group were polished with polishing paste with no desensitization components. Air blast dentin hypersensitivity examinations were performed, and all the patients were asked to grade the sensitivity on a 100-mm visual analogue scale before polishing, immediately after the polishing, 3 days, 1 week, 4 weeks, and 8 weeks after the operation. Hypersensitivity measurement with VAS was recorded. Results SEM observation showed that the surface of teeth was polished and most of the dentin tubules were occluded after polished with Colgate in-office desensitizing paste. The occlusion of dentinal tubules was shallow. The VAS values were significantly different between in-office desensitizing paste group and polishing paste group at just after the polishing, 3 days, 1 week, 4 weeks, and 8 weeks after the operation (P<0.05). Conclusion Colgate in-office desensitizing paste can effectively relieve dentin hypersensitivity in patients with alveolar resorption and osteoporosis after ultrasonic scaling. |
