      目的比较全髋关节置换术采用ABG柄和SECUR~FITHA柄对假体周围骨密度的改变,并分析影响因素。方法对 我院2010年1月至2013年1月的56例(68侧)的全髋关节置换患者,27例(34侧)患者采用ABG柄(史塞克,美国),29例 (34侧)采用SECUR~FIT HA柄(史塞克,美国)进行手术。术后我们采用双能量X线吸收测量法对两种股骨假体柄周围的骨 密度进行测量,并进行比较。假体骨质分区方法采用Gruen法。结果ABG组和SECUR~FIT HA组在1区和7区骨密度改变 均有显著性差异(P <0.05); SECUR-FIT HA组在2、6区骨密度改变同术前相比较有显著性差异(P <0.05)。结论解剖柄 能够减少应力遮挡作用,因此,ABG柄比SECUR~FIT HA柄骨量丢失范围小。

      Objective To compare the change of periprosthetic bone mineral density (BMD) after the total hip replacement with ABG and SECUR-FIT HA femoral component,and to analyze the influential factors. Methods A total of 56 patients (68 hips), who underwent the total hip replacement in our hospital from January 2010 to January 2013,were selected. Among these patients,27 patients (34 hips) underwent the total hip replacement with the ABG stem ( Stryker, American), and 29 patients (34 hips) with SECUR-FIT HA stem (Stryker,American). The periprosthetic bone mineral density ( BMD) of the femur was detected using dual - energy X^ay absorptiometry and compared between the two groups . The bone resorption area was calculated and recorded according to the Gruen ’ s standard. Results The change of BMD in zone 1 and 7 in two groups were significant different (P <0.05). In the SECUR-FIT HA stem group,the change of BMD in zone 2 and 6 was significant different to that before the operation (P < 0. 05). Conclusion The ABG stem can reduce the stress shielding. Thus,the bone resorption area around the ABG stem is smaller than that around the SECUR-FIT HA stem
