| 骨质疏松是糖尿病的并发症之一，故降低血糖的同时也不能忽视对骨质疏松的防治。近年来的一些研究发现，胰高血糖素样肽-1（glucagon like peptide 1，GLP-1）不仅在控制血糖方面发挥重要作用，而且在减缓糖尿病合并骨质疏松方面也有疗效。GLP-1及其类似物可改善机体高血糖状态，抑制骨质吸收；并通过破骨细胞核因子κ B受体活化因子配体（receptor activator for nuclear factor-κ B ligand，RANKL）通路，阻止破骨细胞的成熟，减缓破骨作用；还可调节成骨细胞Wnt通路，影响Wnt／β-catenin比例，促进成骨细胞的分化与成熟，有利于骨质沉积；在间充质干细胞（bone mesenchymal stem cells，BMSC）的分化亦起到调节作用，促使其向成骨细胞分化，减少向脂肪细胞分化；GLP-1及其类似物可以减少晚期糖基化终末产物（advanced glycationend products，AGEs）在骨胶原中沉积，避免破骨细胞的过度增值与分化，此外GLP-1及其类似物还通过其他途径保护骨代谢，减缓骨质疏松的进展。 |

| Osteoporosis is one of the complications of diabetes. There is a need to prevent and cure osteoporosis when reducing blood glucose. Recently, some researches show that glucagon like peptide 1 (GLP-1) not only reduces blood sugar level, but also affects the process of bone turnover. GLP-1 and its analogues can relieve the high blood glucose status, resulting in the inhibition of bone resorption. They can influence RANKL signaling pathway to prevent the maturation of osteoclasts. They can also regulate Wnt signaling pathway to influence osteoblast differentiation and maturation. GLP-1and its analogues regulate differentiation of BMSC to osteoblasts instead of adipocytes. They reduce the production of AGEs in the bone, and inhibit the proliferation and differentiation of osteoclasts, resulting in preventing bone metabolism and slowing down the progression of osteoporosis. |
