目的 肥胖是多种心血管疾病的重要危险因素之一。Roux-en-Y胃旁路(RYGB)术近年来被证实可用于治疗重度肥胖及2型糖尿病。有研究亦指出Roux-en-Y胃旁路术可对患者的骨代谢造成影响,造成骨密度下降。因此,本研究旨在分析Roux-en-Y胃旁路术后1年患者骨代谢的变化,明确Roux-en-Y胃旁路术对中国人群骨密度的影响。方法 回顾性分析16例肥胖患者Roux-en-Y胃旁路术后及对照人群3~12个月时对体重、腹围及身体质量指数(Body Mass Index, BMI)等肥胖指标的影响及对血钙、血磷及25-羟基VitD含量等骨代谢相关指标的变化,并进一步分析RYGB胃旁路术对骨密度的影响。结果 患者体重、腹围及身体质量指数在术后6~12个月明显下降(P<0.05);两组间血钙、磷以及25-羟基VitD含量无明显差异;RYGB组的甲状旁腺素(PTH)在术后第12个月显著升高(P<0.05),RYGB组的骨密度T值在术后6~12个月显著下降(P<0.05)。结论 Roux-en-Y胃旁路术对于肥胖具有明显的改善作用,但同时也能导致骨代谢的紊乱,具有潜在的致骨质疏松风险。 |

Objective To determine the change of bone metabolism after 1-year Roux-en-Y gastric bypass (RYGB). Methods The differences of body weight, abdomen circumference, body mass index (BMI), and bone metabolism parameters were retrospectively analyzed in 16 patients after 3-12-month RYGB and in 23 health controls. The effect of RYGB on BMD was also analyzed. Results The body weight, abdomen circumference, and BMI decreased significantly in 6-12 months after RYGB (P<0.05). Serum levels of calcium, phosphorus, and 25-hydroxyvitamin D were not significantly changed. However, serum PTH in RYGB group increased in 12 months. BMD significantly reduced in RYGB group in 6-12 months. Conclusion RYGB is the effective therapy for obesity, but may lead to a disorder of bone metabolism and risk of osteoporosis. |
