      目的 探讨妊娠糖尿病患者血清N端-骨钙素、总I型胶原氨基端延长肽、β-胶原特殊序列、25-羟基维生素D和甲状旁 腺激素水平的变化。方法 采用病例对照研究,选取妊娠糖尿病患者(GDM组)与同孕周糖耐量正常的孕妇(NGT组)各50 例,测定N端-骨钙素、总I型胶原氨基端延长肽、β-胶原特殊序列、25-羟基维生素D和甲状旁腺激素水平,比较两组人群临床资料和骨代谢标志物水平的差异并进行统计学分析。结果 GDM组孕妇的血清总I型胶原氨基端延长肽为(47. 87 ±34. 84) μg /L,明显高于NGT组的(35.48±10.61)| μg /L (P=0.018),血清β-胶原特殊序列为(0. 226 ±0. 095 ) μg /L ,明显高于NGT组 的(0. 193 ± 0. 054) μg /L(P = 0. 038),血清 N 端-骨钙素为(9. 82 ± 4. 74) μg/L,明显高于 NGT 组的(7. 74 ± 1. 92) μ g/L( P = 0.005。25-羟基维生素D和甲状旁腺激素水平两组间差异无统计学意义。结论 GDM组患者的骨代谢状态主要表现为骨合成代谢和骨吸收代谢均有升高,同时骨转换率也明显升高,尚未发现维生素D和甲状旁腺激素水平与糖耐量正常的孕妇之间有明显差异。

      Objective To investigate the changes of serum N-MID osteocalcin, total procollagen type 1 N-terminal propeptide, β- crosslaps, vitamin D, and parathyroid hormone in gestational diabetes mellitus. Methods Using case control study, 50 gestational diabetic patients (GDM group) and 50 pregnant women with normal glucose tolerance (NGT group) were included in this study. The levels of serum total procollagen type 1 N-terminal propeptide, N-MID osteocalcin, β-crosslaps, vitamin D, and parathyroid hormone were detected and compared. Results Serum levels of total procollagen type 1 N-terminal propeptide, N-MID osteocalcin, and β-crosslaps in GDM group (47. 87 ±34.84) μg/L,9. 82 ± 4. 74μg/L, and 0. 226 ± 0. 095μg/L, respectively) were significantly higher than those in NGT group (35.48 ± 10,61μg/L, 7.74 ± 1.92μg/L,and 0. 193 ±0.054μg/L, P = 0. 018,P =0. 038,and P =0. 005,respectively). No statistical difference of 25OHD3 and parathyroid hormone was found between the two groups. Conclusion Bone formation level, bone resorption level, and turnover ratio in gestational diabetic patients are higher than pregnant women with normal glucose tolerance. No obvious difference of vitamin D and parathyroid is found between them.
