骨创治疗仪治疗原发Ⅱ型骨质疏松症患者肋骨骨折的疗效观察

      目的 探讨骨创治疗仪对于原发Ⅱ型骨质疏松症患者肋骨骨折的临床疗效。方法 参照临床流行病学(DME)原则,纳人符合标准的原发Ⅱ型骨质疏松症合并肋骨骨折受试者43例,随机分为观察组(21例,其中男8例,女13例,平均年龄 65.7岁,平均体重67.3kg)、对照组(22例,其中男7例,女15例,平均年龄66.2岁,平均体重68. 1 kg),两组均给予基础药物治疗:钙尔奇D片、骨化三醇胶丸,分别给予肋骨带制动或肋骨带制动联合骨创治疗仪治疗,在治疗前、后检测患者肋骨正斜位DR、右髋骨密度、血清骨碱性磷酸酶,由2名专业医师对患者的疼痛程度评分进行评定。结果 基线比较,组间比较均无差异(P >0.05)。疗效结果统计显示:观察组患者干预后疼痛评分较干预前明显降低(P <0.01),较对照组亦明显降低(P<0.01),观察组右髋BMD干预后较干预前有升高趋势(P>0.05) ,BALP组内比较观察组干预前后显著提高,对照组干预前后无统计学差异(P>0. 05);观察组CTX干预后较干预前明显降低(P<0.01),与对照组干预后比较观察组CTX亦明显降低,提示干预后观察组骨形成增加,骨吸收减少。观察组干预后GH、VT、SF、MH明显提高,与对照组干预后比较,观察组VT、MH提高具有统计学差异(P<0.05),提示干预后精力及精神健康状况明显好转。结论 骨创治疗仪能够降低骨转换,但短期内对患者骨密度未见明显影响,对老年性肋骨骨折具有良好的止痛作用,有利于提高患者精力、社会功能、精神健康状态以及生活自理能力。

      Objective To explore the efficacy of bone therapeutic apparatus on the treatment of rib fractures in patients with primary type II osteoporosis. Methods According to the principle of clinical epidemiology (DME),43 cases with rib fractures who met the standard of primary osteoporosis type II were included. They were randomly divided into observation group (21 cases, 8 males and 13 females, with an average age of 65.7 years and an average weight of 67. 3 kg) and the control group (22 cases, 7 males and 15 females, with an average age of 66.2 years and an average weight of 68.1 kg). Patients in the two groups received basic therapy: including Caltrate D and Calcitriol. They were treated with rib with brake or rib with brake combined with bone therapeutic apparatus, respectively. Before and after the treatment, oblique rib X-ray, bone mineral density of the right hip, and serum bone alkaline phosphatase were detected. The score of the pain degree of the patients was assessed by 2 professional doctors. Results At baseline, there was no difference between the 2 groups (P >0.05). The results showed that the pain score in the observation group was significantly lower than that in the control group (P<0. 01).BMD of the right hip in the observation group showed increased trend after the treatment compared to that before the intervention (P > 0. 05). BALP increased in the observation group after the intervention but it did not change in the control group (P >0.05). CTX decreased significantly in the observation group and it was lower than that in the control group (P < 0. 01),indicating that bone formation increased and bone resorption decreased in the observation group. GH, SF, VT, and MH significantly improved in the observation group, and the increase were statistically different compared to the control group (P < 0. 05), indicating that the mental health status in the intervention group was significantly improved. Conclusion Bone invasive therapy can reduce bone turnover, but it has no significant effect on bone mineral density in the short term. It has a good analgesic effect on senile rib fractures, which is beneficial to improve the patient’s energy, social function, mental health status,and self-living ability.
