目的 探讨2型糖尿病患者血清25-(OH)D水平与体脂分布的相关性。方法 选取2014年3月至2014年10月在我科住院的2型糖尿病患者86例,其中绝经后女性62例,50岁及以上男性24例,对其临床基本资料及生化指标、血清25-(OH) D和体脂成分等进行测定。按血清25-(OH)D水平分为不同的亚组,即严重缺乏组(25例)、缺乏组(44例)、不足组(17例), 比较各组体脂分布是否存在差异。采用Pearson相关系数评价各体脂指标的相关性。采用多元逐步线性回归方法分析血清 25-(OH)D等指标与体脂指标的相关性。结果 25-(OH)D严重缺乏组个体其内脏脂肪与皮下脂肪的比值(VAT/SAT)高于 25-( 0H)D不足组(P <0.05), 25-(OH)D缺乏组个体其BMI高于25-( OH) D不足组(P < 0. 05)。VAT/SAT与腹臀比、躯干脂 肪质量/四肢脂肪质量、躯干脂肪百分比/腿部脂肪百分比均显著相关(P值均<0.05)。BMI与除外VAT/SAT的其余各体脂 指标均显著相关(P值均<0.05)。多元逐步线性回归分析示绝经后女性患者血清25-(OH)D水平与VAT/SAT显著相关(P <0.01)。结论2型糖尿病患者血清25-(OH)D水平与体脂分布相互关联,且绝经后女性患者更为显著。 |

Objective To investigate the relationship between serum 25-(OH) D level and body fat distribution in patients with type 2 diabetes mellitus. Methods Eighty-six patients hospitalized in our department with type 2 diabetes mellitus from March 2014 to October 2014, including 62 post-menopausal women and 24 men over the age of 50 years old. The basic clinical information, biochemical parameters, serum 25-(OH) D, body fat composition, and other parameters were measured. According to serum 25- (OH) D level, the 86 cases were divided into severe deficiency group (25 cases),insufficiency group (44 cases),and deficiency group (17 cases). The differences between the different body fat indexes among the groups were analyzed using Pearson correlation method. The stepwise multiple linear regression was involved in analyzing the relationship of biochemical parameters, serum 25- (OH) D, and body fat indexes. Results The VAT/SAT ratio in severe deficiency group was higher than that in deficiency group (P < 0.05). BMI in insufficiency group was higher than that in deficiency group ( P < 0. 05 ). VAT/SAT was significantly correlated with A/G, TF% /LF% , and TFM/LFM (P<0.05).BMI was correlated with BF% , A/G, TF% /LF% , and TFM/ LFM (P < 0. 05). Stepwise multiple linear regression showed that the level of 25-(OH) D was significantly associated with VAT/ SAT in postmenopausal women ( P < 0. 01). Conclusion The 25-( OH) D level of patients with type 2 diabetes mellitus is correlated with body fat distribution, and it is more significant in postmenopausal women. |
