目的 通过比较H型高血压、普通高血压和血压正常人群骨密度(Bone Mineral Density, BMD)情况,探讨H型高血压对BMD的影响。方法 对绵阳地区城市、郊区及农村1215例70岁以上男性自愿者进行腰椎、股骨颈和股骨转子区BMD检测,符合标准纳人标准的有813人。骨密度按世界卫生组织(WHO)标准分类:骨量正常、骨量减少和骨质疏松。对参与者血压和血清同型半胱氨酸(Homocysteine, HCY)进行检测,根据中华医学HCY和血压的分类标准,高血压同时伴有血清HCY升高称为H型高血压。参与者血压情况分为三类:H型高血压患者、普通高血压患者及血压正常人群。比较三类人群BMD值、骨量减少和骨质疏松患病率。结果 H型高血压患者共有75例,普通高血压患者107例,正常人群631例。H型高血压患者BMD值显著低于普通高血压患者和血压正常人群,骨量减少和骨质疏松患病率显著高于普通高血压患者和血压正常人群(P <0. 05),普通高血压患者BMD值显著低于普通血压正常人群,骨量减少和骨质疏松患病率显著高于血压正常人群(P< 0.005)。结论 老年男性中,高HCY和高血压对BMD的影响有交互作用,两者同时存在时BMD显著降低,骨量减少和骨质疏松显著升高。 |

Objective To investigate effect of H-hypertension on bone mineral density (BMD) in elderly males, by comparing BMD among H-hypertensive patients, hypertensive patients, and normal population. Methods BMD of the lumbar spine, femoral neck,and femoral trochanter was detected in 1215 male volunteers aged over 70 years in urban, suburban and rural area of Mianyang. BMD in 813 people who met the criteria was analyzed. According to the World Health Organization (WHO) standard classification, participants were divided into normal bone mass, osteopenia, and osteoporosis. Blood pressure and serum homocysteine (HCY) were tested. According to Chinese Medical Association classification of HCY and blood pressure standard, participants were divided into H-hypertensive patients (hypertension with the increase of HCY) , hypertensive patients and normal population. BMD and prevalence of osteopenia and osteoporosis were compared among the 3 groups. Results There were 75 cases of H-hypertension, 107 cases of hypertension, and 631 of normal blood pressure. Compared with hypertensive patients and normal pressure patients, BMD in H-hypertensive patients was significantly lower,but prevalence of osteopenia and osteoporosis was significantly higher (P < 0. 05). Compared with normal population, BMD in hypertensive patients was significantly lower, but prevalence of osteopenia and osteoporosis was significantly higher ( P < 0. 05 ). Conclusion In elder men,high HCY and hypertension interacts with BMD and causes the decrease of BMD and the increase of the prevalence of osteopenia and osteoporosis. |
