      目的 探讨慢性乙型肝炎(Chronic hepatitis B,CHB)与骨质疏松症之间的相关关系。方法 分别对 201 例 CHB 患者(CHB 组)和 122 例健康体检者(对照组)进行和左侧股骨颈和腰椎前后位(L1-4 椎体)的骨密度(Bone mineral density,BMD)进行测定。对两组的 BMD、骨质疏松患病率进行比较,并对 BMD 的影响因素进行多元逐步回归分析。结果 (1)男性及女性受试者中,CHB 组和对照组的患者的左侧股骨近端多个部位和多个腰椎 BMD 比较差异有显著性(P < 0. 05)。(2)在男性受试者中,CHB 组中骨质疏松症患病率高于正常对照组,差异有统计学意义(P <0. 05)。(3)在小于 40 岁、40 岁至 60 岁、大于60 岁的不同年龄段男性及女性受试者中,CHB 组和对照组的患者的左侧股骨近端多个部位和多个腰椎 BMD 比较差异有显著性(P <0. 05)。(4)线性多元逐步回归分析显示,以 CHB 患者的左股骨近端 BMD 为因变量,BMD 与 CHB 病程呈负相关(r =-0. 52,P <0. 05)。以 CHB 患者的腰椎 BMD 为因变量,BMD 与 CHB 病程呈负相关(r = -0. 28,P <0. 05)。结论 CHB 可能是骨质疏松症的危险因素之一。

      Objective To evaluate the relationship between chronic hepatitis B (CHB) and osteoporosis. Methods BMD of the left femoral neck and lumbar vertebrae L1-4 were measured in 201 patients with CHB and 122 healthy people (control group). The BMD and the prevalence of osteoporosis between the two groups were compared. Multivariate regression analysis was used to analyze the risk factors of BMD. Results (1) In both male and female subjects,the BMD of left proximal femur and the lumbar spine was significantly different between CHB group and control group (P < 0. 05). (2) In male subjects,the prevalence of osteoporosis in CHB group was higher than that in the control group,and the difference was statistically significant (P < 0. 05). (3) In both male and female subjects at age of less than 40 years old,between 40 to 60 years old,and more than 60 years old,the BMD of the left proximal femur and lumbar spine was significantly different between CHB group and control group (P < 0. 05).(4) Multiple linear stepwise regression analysis showed that in patients with CHB,BMD of the left proximal femur was negatively correlated with CHB course (sr = -0. 52,P <0. 05),and BMD of the lumbar spine was negatively correlated with CHB course (r= -0. 28,P <0. 05). Conclusion CHB may be a risk factor for osteoporosis.
