      目的 分析绝经前后低骨密度、高骨密度女性外周血单核细胞基因表达谱,探讨外周血单核细胞差异表达基因对低骨密度、高骨密度人群的鉴别诊断价值。方法 下载公众数据平台美国国家生物技术信息中心(National Center of Biotechnology Information,NCBI)下的基因芯片原始数据集NCBI-GEO DataSets-GSE56815 ,运用R/Bionconductor分析原始数据,选取差异倍数> 1. 5,P <0. 05的基因为差异表达基因;运用Medcalc及SPSS统计软件对差异表达基因绘制受试者工作特征曲线,分析单一基因检测及联合基因检测对低骨密度、髙骨密度人群的鉴别诊断效果。结果 在绝经前女性中,低骨密度、高骨密度组外周血单核细胞存在着58个差异基因(18基因上调、40基因下调),且FOLR3&ITGB3&ABO&IGHM联合检测鉴别诊断不同骨密度女性有较好的特异性、敏感性;在绝经后女性中,低骨密度、高骨密度组外周血单核细胞存在着75个差异基因(21基因上 调,54基因下调),且TACSTD2&MMP8&LILRA5联合检测鉴别诊断不同骨密度女性有较好的特异性、敏感性。结论 绝经前后女性外周血单核细胞基因表达谱不同,低骨密度组与高骨密度组外周血单核细胞基因表达谱在绝经后女性中差异变化更为显著,外周血单核细胞差异基因联合检测能够提高对不同骨密度女性的鉴别诊断能力。

      Objective To investigate the gene expression profile of circulating monocytes in pre- and post-menopausal females with low and high bone mineral density (BMD), and to explore the value of differentially expressed genes in differentiating low BMD from high BMD in pre- and post-menopausal females. Methods GSE56815 dataset was derived from NCBI-GEO DataSets ;R/Bionconductor was performed for gene expression analysis. Genes with fold change > 1.5 & P < 0. 05 were selected as differentially expressed genes (DEGs). Medcalc and SPSS statistical software was utilized for receiver operating characteristic (ROC) curve evaluation of single DEGs detection and combined DEGs detection. Results A total of 58 DEGs (18 up-regulated, 40 down-regulated ) existed in circulating monocytes between pre-menopausal females with low and high BMD. FOLR3&ITGB3&ABO&IGHM combined analysis could improve the differential diagnosis between low and high BMD in pre?menopausal females. A total of 75 DEGs (24 up-regulated, 51 down-regulated) existed in circulating monocytes between post-menopausal females with low and high BMD. TACSTD2&MMP8&LILRA5 combined analysis could improve the differential diagnosis between low and high BMD in post-menopausal females. Conclusion The gene expression profiles of circulating monocytes were different in pre- and post-menopausal females, and there were more DEGs of low BMD in post-menopausal subjects. Combined detection of circulating monocytes DEGs could improve the differential diagnosis capability for female with different BMD.
