目的 探讨骨形成指标I型胶原氨基端肽原(procollagen type I N-terminal propeptide, PIMP)与骨吸收指标β-异构C-端肽(beta-isomerized C-telopeptide, β-CTX)的表达在 2 型糖尿病(type 2 diabetes, T2DM )合并骨质疏松(osteoprosis, OP)中的意义。方法 将118例研究对象按双能X线检测骨密度(bone mineral density,BM D)的检杏结果分为3组,骨量正常组41例,骨 量减少组42例,骨质疏松组35例。记录其性别、年龄、身高、体重、体质指数(body mass index, BMI)、糖尿病病程等一般基线资料。使用全自动生化分析仪测定空腹血糖(fasting blood glucose,FBG)、血钙(blood calcium,Ca)、血磷(phosphate,P)、血清胆固醇(total cholesterol, TC )、甘油三酷(triglyceride, TG )、碱性磷酸酶(alkaline phosphatase, ALP )、甲状旁腺素(parathyroid hormone,PTH)等生化指标,高压液相法测定糖化血红蛋白(HbAlc),ELISA法检测血清骨形成指标P1NP及血清骨吸收指标 (β-CTX并进行组间比较。结果 与骨量正常组比较,骨量减少组与OP组中的PINP、β-CTX均高于正常组(P<0.05),与骨量减少组相比,OP组中PINP、β-CTX均高于减少组(P<0.05)。股骨颈BMD与年龄、病程、FPG、HbAl c、β-CTX、PINP呈负相关 (r= -0.559、-0.368、-0.255、-0.284、-0.613、-0.314, P <0.05),与 BM1 无明显相关性(P > 0. 05 )。结论 T2DM 合并OP患者中骨形成与骨吸收均增高,属于高转换性的骨代谢,监测骨代谢标志物PINP与β -CTX.有助于早期诊断及防治T2DM合并OP的发生与发展。 |

Objective To investigate the significance of the expression of bone formation index procollagen type I N-terminal propeptide (PINP) and bone resorption index beta-isomerized C-telopeptide (beta-CTX) in patients with type 2 diabetes mellitus (T2DM) and osteoporosis (OP). Methods According to the results of dual energy X-ray BMD examination,118 subjects of T2DM were divided into normal bone mass group (41 cases),osteopenia group (42 cases), and OP group (35 cases). The general clinical baseline data, such as gender, age, height, weight, mass index body, and duration of diabetes, were recorded. Biochemical indexes fasting blood glucose (FBG),calcium (CA) , phosphorus (P),serum total cholesterol (TC),triglyceride (TG),alkaline phosphatase (ALP),and parathyroid hormone (PTH) were determined using an automatic biochemical analyzer. Glycosylated hemoglobin (HbAlc) was determined using high pressure liquid chromatography method. Serum biochemical markers PINP and beta-CTX were detected using ELISA method. Results PINP and beta-CTX in osteopenia group and the OP group were higher than those in normal bone mass group (P < 0. 05). PINP and beta-CTX in OP group were higher than those in osteopenia group (P <0. 05). BMD of the femoral neck was negatively correlated with age, course of disease, FPG, HbAlc, PINP, and beta- CTX (r = -0. 559, -0. 368, -0. 255,-0. 284, -0. 613,and -0. 314, respectively,P <0. 05),but was not correlated with BMI. Conclusion Both bone resorption and bone formation increase in patients with T2DM and OP, showing a high conversion of bone metabolism. Timely monitor the bone metabolism markers PINP and beta-CTX can help for early diagnosis and prevention the T2DM with OP. |
