| 目的 评价基于数字化平台的持续化骨质疏松健康管理对生活习惯、药物依从性的长期影响。方法 选取解放军第 309医院骨科中心门诊2013年8月至2014年3月的274例骨质疏松症患者，随机分为研究组（137例）和对照组（137例）。研究组患者给予基于数字化平台的持续化骨质疏松健康管理，对照组于门诊常规随诊，随访期限2年。对比分析两组患者在不同阶段的良好生活习惯（包括喝牛奶、户外接触阳光、足量运动、不吸烟、不饮酒）和良好药物依从性（包括钙剂、维生素D及双膦酸盐类药物）的人数比例。结果 研究组及对照组患者在随访1年时各项良好生活习惯的人数比例都分别较入院时有明显的增加（P<0. 05),但研究组比对照组增加更显著（P< 0.01);在随访2年时对照组各项良好生活习惯的人数显著减少 (P <0.05),而研究组没有明显变化（P >0.05),各项良好生活习惯的人数比例均显著髙于对照组（P <0.01)。研究组及对照组患者在短期内（0?6个月）钙剂、维生素D及双膦酸盐的药物依从性良好的人数比例差异均没有统计学意义（P >0.05)。 随着时间的推移，在6?12个月、12?24个月时，两组患者各种药物依从性良好人数均呈逐渐减少的趋势（P <0. 05),但对照组比研究组减少更为显著（P <0.01)。结论 利用基于数字化平台的持续化骨质疏松健康管理,可以有效提高患者良好生活习惯、服药的长期依从性。 |

| Objective To evaluate the long-term effect of sustaining osteoporosis health management based on the digital platform on adherence of life style and drugs. Methods A total of 274 osteoporosis patients, collected in the orthopedic center outpatient of the 309 Hospital of PLA from August 2013 to March 2014，were randomly assigned to research group (n = 137) and control group (n = 137). The patients in research group were intervened by sustaining osteoporosis health management based on the digital platform, while the patients in control group were followed-up in the outpatient. The follow-up period lasted for 2 years. The adherence of good life style (drinking milk, sufficient outdoor sunshine, adequate exercise, no smoking, and no alcohol intake) and medication (calcium, vitamin D, and bisphosphonates) were evaluated in different period. Results Compared with admission, the number of patients who had good behaviors in research group and control group at one year of follow-up increased significantly (P <0.05), And the patients in research group with good behaviors increased more than those in control group (P <0. 01). At two year of follow-up, the number of patients with good behaviors in control group decreased significantly (P < 0. 05) , but those in research group had no significant change (P > 0. 05 ). And the patients with good behaviors in research group were significantly more than those in control group (P <0. 01).The number of patients who had good medicine adherence of calcium, vitamin D, and bisphosphonates in short term (0-6 months) was not significantly different between research group and control group (P >0.05). In 6-12 months and in 12 - 24 months, the number of patients in both two groups with good adherence of the three drugs had gradually decreasing trend (P <0.05). However, the patients with good medicine adherence in control group decreased more than those in research group (P < 0. 01). Conclusion The sustaining osteoporosis health management based on the digital platform improves adherence of good behaviors and drugs. |
