      目的 研究人体脊柱松质骨骨骼显微结构和力学性能的区域性差异。方法 采集6个人颈椎C5椎体标本,进行显微CT扫描,应用结构参数测量、显微有限元分析、力学参数计算方法,研究人椎体内解剖部位对于结构参数和力学参数测量结果的影响,应用配对t检验比较结构参数和松质骨的表观刚度在不同部位的差异(α=0.05)。结果 腹侧和背侧的结构参数仅有杆状骨小梁体积分数(rBV/TV), R-R 接头密度(R-R junc), 横向骨小梁体积分数(tBV/TV)存在差异,外侧和内侧的结构参数仅有平均骨小梁厚度(pTb.Th), 杆状骨小梁平均厚度(rTb.Th), P-P 接头密度(P-P Junc), R-P 接头密度(R-P Junc), 纵向骨小梁体积分数(lBV/TV)不存在差异,头侧和尾侧的结构参数均存在明显差异。代表骨质增多的指标,在尾侧均大于头侧,比如骨体积分数(BV/TV),杆状骨小梁体积分数(rBV/TV),板状骨小梁组织分数(pBV/TV),骨小梁厚度(Tb.Th),骨小梁数目(Tb.N)。腹侧和背侧的表观弹性模量没有差异,表观剪切模量存在差异;外侧和内侧的表观弹性模量在非主方向上存在显著差异,在主方向上无显著差异,表观剪切模量无显著差异;头侧和尾侧的表观弹性模量在主方向上存在显著差异。结论 ①在颈椎椎体松质骨,头侧和尾侧的结构参数均存在显著差异,其余部位的结构参数无显著差异。②在颈椎椎体松质骨,头侧和尾侧的表观弹性模量在主方向上存在显著差异;此外外侧和内侧的表观弹性模量在非主方向上存在显著差异,在主方向上无显著差异,表观剪切模量无显著差异。

      Objective To study the regional differences in the microstructure and mechanical properties of the cancellous bone of the human spine. Methods Six human C5 body samples were scanned with micro-CT. The measurement of architectural parameters, large-scale finite element analysis, and calculation of mechanical parameters were performed. The effect of the anatomical site on architectural parameters of the vertebral body was studied. The difference of the structure parameters and cancellous bone apparent stiffness among different parts was compared using the paired t test (α=0.05). Results There were differences in rBV/TV, R-R junc, and tBV/TV between the ventral and dorsal sides. There were no differences in pTb.Th, rTb.Th, P-P Junc, R-P Junc, and lBV/TV between the lateral and medial sides. The indexes of bone growth in the tail side were greater than those in the head side. The apparent elastic modulus was not different between ventral and dorsal side, but the apparent shear modulus was different between ventral and dorsal side. The apparent elastic of modulus was remarkably different between lateral and medial side in the non-main direction, but there was not significantly different in the main direction. The apparent shear modulus was not significantly different. There was significant difference in apparent elastic modulus between rostral and caudal side in the main direction. Conclusion ①There are significant differences in the structural parameters of the cancellous bone in the cervical vertebral body between the head and tail, but there was no significant difference of the structural parameters in the other parts. ②The apparent elastic modulus is significantly different between cranial and caudal side in the main direction of the cancellous bone in the cervical vertebrae. There are remarkable differences of the apparent elastic modulus between the lateral and medial side in the non-main direction, while there is no significant difference in the main direction. There is no significant difference in the apparent shear modulus.
