      目的 探讨ADAMTS-7与ONFH(股骨头坏死)患者关节软骨退化和影像学变化的相关性,并研究其潜在机制。方法 共纳人2014年3月至2015年6月我院收治的80例股骨颈骨折(FNF)患者作为研究对象。根据检查结果将所有患者分为对照组和ONFH组,对照组患者X线片检查均无ONFH和骨关节炎OA;ONFH组患者X线片检查有明确ONFH。根据ARCO分期标准,将ONFH组患者进一步分为两组:A组,ARCO分期II ~ III期,X线片显示股骨头无明显塌陷;B组,ARCO分期IV期, X线片显示股骨头有明显塌陷。记录Mankin评分;免疫组化染色分析ADAMTS-7、TNF-α和磷酸化NF-κB的表达水平;RT- PCR检测ADAMTS-7的mRNA水平;Western Blot检测ADAMTS-7与COMP的蛋白水平;分析各组指标差异,探讨ADAMTS-7 在ONFH发病机制中的潜在作用。结果 各组患者的年龄、性别等基本信息对比无统计学差异(P >0. 05)。ONFH组患者关节软骨发生明显退化,ONFH A组与ONFH B组的Mankin评分均显著高于对照组(P <0. 05)。ONFH组的ADAMTS-7和TNF- α表达水平均显著高于对照组(P <0.05)。ONFH组ADAMTS-7的mRNA水平显著高于对照组(P < 0. 05 )。Western Blot结果显示ADAMTS-7的增加与COMP的降解潜在相关(P <0. 05)。此外,ONFH组患者的NF-κB表达水平显著高于对照组(P < 0. 05)。结论 ONFH患者关节软骨ADAMTS-7 ,TNF-α与磷酸化NF-κB均显著上调,ADAMTS-7的表达水平与软骨TNF-α和磷酸化NF-κB P65的表达呈正相关,ADAMTS-7可能通过TNF-α 与磷酸化NF-κB P65参与ONFH的发病机制。

      Objective To study the relationship between ADAMTS-7 and cartilage degradation and radiological change in patients with osteonecrosis of femoral head (ONFH) , and to investigate the potential mechanism. Methods A total of 80 cases of patients with femoral neck fracture (FNF) in our hospital from March 2014 to June 2015 were recruited. Human hip articular cartilage specimens were collected from patients with FNF without ONFH and OA in X-ray (healthy control group) and patients with ONFH (ONFH group). According to ARCO classification standard, articular cartilage specimens obtained from the ONFH patients were categorized into 2 groups : group A,stage II-III with no obvious collapse in X-ray;and Group B, stage IV with obvious femoral head collapse in X-ray. Mankin scores were recorded. The expression levels of ADAMTS-7, TNF-α, and phospho-NF-κB were measured with immunohistochemistry. The mRNA level of ADAMTS-7 was measured with RT-PCR,Western blotting was used to evaluate the protein levels of ADAMTS-7 and COMP. Data were compared among different groups. The potential role of ADAMTS-7 in ONFH pathogenesis was explored. Results There were no significant differences in age, gender, and other basic information among the different groups (P > 0.05). Articular cartilage was degraded in ONFH patients. Mankin score both in ONFH A and B groups were significantly higher than that in control group (P<0. 05). The immunohistochemistry results revealed that the levels of ADAMTS-7 and TNF-α increased in degraded cartilage of ONFH group ( P < 0. 05). The expression of ADAMTS-7 in ONFH group increased significantly compared to that in control group (P <0.05). Western blotting result showed positive association between increased ADAMTS-7 and COMP degradation in ONFH patients (P < 0. 05). Furthermore, the expression of phospho-NF-κB in ONFH groups was significantly higher than that in the control group (P <0. 05). Conclusion The expressions of ADAMTS-7, TNF-α, and phospho-NF-κB are significantly upregulated in the articular cartilage of ONFH patients. ADAMTS-7 may affect ONFH pathogenesis through TNF-α and phospho-NF-κB.
