      目的 分析老年男性骨密度(bone mineral density,BMD)与身体成分的关系。方法 记录166名80 ~94岁老年男性的年龄、身髙、体重、BMI,检测L1-4、左侧股骨颈、Ward三角、大粗隆、左侧股骨上端的BMD及总肌肉含量(total lean mass,TLM) 和总脂肪含量(total fat mass,TFM),根据BMD水平将受试者分为骨量正常组(n =50)、骨量低下组(n =91)和骨质疏松组(n = 25)。结果 ①骨量低下组及骨质疏松组的BMD均明显低于正常骨量组(P <0.01),骨质疏松组又明显低于骨量低下组 (P <0. 01);②骨质疏松组体重、TLM、TFM及BMI均明显低于正常骨量组(P <0. 01) ;③TLM与左侧股骨颈、Ward三角、左侧 股骨上端的BMD相关系数分别为0. 227、0. 203及0. 193( P <0. 05),TFM与腰椎1-4、左侧股骨颈、Ward三角、大粗隆、左侧股骨上端的BMD相关系数为0. 269、0. 222、0. 206、0. 246及0. 242(P <0. 01)。结论 大部分老年男性BMD与年龄呈显著负相关:与体重、BMI及TFM、TLM呈显著正相关,适当的运动和适度的肥胖,增强肌肉量和维持一定的脂肪量,保持健康的体重,可能有利于预防老年性骨质疏松症。

      Objective To study the relationship between bone mineral density and body composition in old men. Method Age, height, weight and BMI were recorded and BMD of lumbar spine 1 to 4,left femoral neck, Ward’s triangle,greater trochanter, left proximal femur and total lean mass (TLM) and total fat mass (TFM) were measured in 166 elderly male aged 80 to 94 years. The subjects were divided into normal bone mass group (n =50),osteopenia group (n =91) and osteoporosis group (n =25) according to the BMD levels. Results ①The BMD of osteopenia group and osteoporosis group were significantly lower than that in the normal bone mass group (P <0. 01) and osteoporosis group had significantly lower BMD than the low bone mass group (P <0. 01).②Body weight, TLM,TFM and BMI of the osteoporosis group were significantly lower than that of the normal bone mass group (P <0. 01). ③The correlation coefficient (r) of TLM and the BMD of left femoral neck,Ward’s triangle, left proximal femur was 0. 227,0. 203 and 0.193, respectively (all P <0. 05), and the correlation coefficient (r) of TFM and the BMD of lumbar spine 1 to 4,left femoral neck,Ward’s triangle,greater trochanter and left proximal femur was 0. 269,0. 222,0. 206 , 0. 246 and 0. 242,respectively (all P < 0.01). Conclusion The correlation of BMD with age was significantly negative in elderly men. The correlations of BMD with weight,BMI and TFM and TLM were significantly positive in elderly men,so maintain proper exercise and moderate obesity,enhance muscle mass,maintain certain amount of fat and a healthy body weight could be beneficial to the prevention of osteoporosis.
