      目的 探讨腰椎间盘突出对绝经后妇女骨密度的影响。方法 随机选择绝经后妇女进行问卷调査,内容包括身髙、体重、是否有腰椎间盘突出、生活习惯、健康状况等;用双能X线骨密度仪测定腰椎、髋部骨密度,并利用SPSS 20.0统计软件对数据进行分析。结果 共筛选人组582例,腰椎间盘突出组133例、腰椎间盘无突出组449例。对各因素与骨质疏松的相关性分析,腰椎间盘突出与腰椎椎体骨密度具有负相关(r = -0. 094,P =0. 023);两组一般资料比较,日光照射、腰椎骨密度差异比较有统计学意义(P <0.05);协方差分析,腰椎间盘无突出组腰椎骨密度(0.734 ±0. 123)g/cm2明显髙于腰椎间盘突出组腰椎骨密度(0. 707 ±0. 123)g/cm2,差异有统计学意义(F=3.968,P = 0.047),髋部骨密度差异无统计学意义(P >0.05);两组 骨质疏松患病率比较,腰椎间盘突出组为55. 6%,腰椎间盘无突出组为43. 7%,两组比较差异有统计学意义(P =0. 015)。 结论 腰椎间盘突出能够引起腰椎骨量流失,是引起骨质疏松发生的影响因素。

      Objective To investigate the effect of lumbar disc herniation on bone mineral density in postmenopausal women. Methods A questionnaire survey was performed in the randomly selected postmenopausal women. It includes the height,lumbar disc hernia history,living habits,health status,etc. Bone mineral density (BMD) of the lumbar spine and hip was measured using dual energy X-ray absorptiometry. A SPSS20. 0 statistical software was used to analyze the data. Results A total of 582 patients were screened,including 133 cases of lumbar intervertebral disc herniation and 449 cases without herniation. Analysis on the correlation between all factors and osteoporosis showed that lumbar disc herniation was negatively correlated with BMD of the lumbar vertebrae (r = -0. 094,P =0. 023). Comparing the general information between the two groups,sunlight and lumbar BMD were different,and the difference was statistically significant (P <0. 05). Covariance analysis showed that BMD of no prominent group (0. 734 ±0. 123 g/cm2) was significantly higher than that of lumbar disc hernia group (0. 707 ±0. 123 g/cm2),and the difference was statistically significant (F = 3. 968,P =3. 968). There was no difference in hip BMD (P >0.05). Comparing the osteoporosis prevalence between the two groups,it was 55. 6% in lumbar disc hernia group and 43. 7% in no prominent group,and the difference was statistically significant (P =0. 015). Conclusion Lumbar disc herniation can cause lumbar bone mass loss,and it is the influence factor of osteoporosis.
