补肾壮骨颗粒对SAMP6小鼠股骨宏观结构及生物力学的影响

      目的 研究中药复方补肾壮骨颗粒对SAMP6小鼠股骨结构及生物力学性能的影响,并探讨是否有可能替代生长激素进行对老年男性骨质疏松的治疗。材料方法 将30只3月龄SAMP6小鼠随机均分为模型组、生长激素组和中药组,选取10只正常老化小鼠(SAMR1)作为同源对照组。正常饲养1周后,生长激素组和中药组小鼠开始给药,每周称重一次调整给药剂量,正常组和模型组给予等量的生理盐水。3个月后每组随机选取5只小鼠去眼球取血处死,取其左侧股骨,进行测定股骨的骨量、几何尺寸、骨截面力学几何量和生物力学性能实验,6个月后采取同样的方法处理剩余的小鼠。结果 与模型组比较,3个月后用药组骨量均降低(P >0.05),6个月后均升高(P>0.05),且中药组均比同期生长激素组骨量高(P >0.05);3个月后中药组SAMP6小鼠皮质骨厚度、截面积增大(P <0.05),骨髓腔直径、横截面积减小(P <0.05),骨截面力学几何量增加(P>0.05),骨结构力学各参数均显著性提高(P<0.01),最大应力、弹性应力显著性提高(P <0.01),刚性系数增大(P <0.05),弹性模量增加(P >0.05);6个月后中药组SAMP6小鼠皮质骨厚度、横截面积增大(P <0.01),骨髓腔直径、横截面积减小(P <0.05),骨的截面力学几何量均显著性提高(P <0.01),最大载荷、弹性载荷提高(P <0.05),最大绕度、最大弯矩、能量吸收提高(P >0.05),弹性应力、刚性系数增加(P >0.05),弹性模量降低(P >0.05);与生长激素组比较,3个月后中药组SAMP6小鼠各检测指标结果无明显差异(P >0.05),6个月后中药组SAMP6小鼠皮质骨厚度、横截面积显著性增大(P <0.01),骨截面力学几何量增加(P>0.05),骨生物力学性能参数结果减小,但无统计学意义(P >0.05)。结论 补肾壮骨颗粒可以调节SAMP6小鼠骨宏观几何结构的力学应答机能,改善骨的生物力学性能,增加骨质韧性,增强骨强度,提高抵抗外力变形和骨折的能力;长期用药可提高SAMP6小鼠骨量,以上结果与生长激素治疗SAMP6小鼠骨质疏松的疗效相当,故补肾壮骨颗粒有可能替代价格昂贵且副作用较多的生长激素来治疗老年男性骨质疏松。

      Objective To study the effects of Bushen Zhuanggu Granules on structure and biomechanical properties of the femur of senescence accelerated male mice(SAMP6) , and to explore whether it would replace growth hormone for the treatment of osteoporosis in elderly men. Methods 30 senescence accelerated male mice(SAMP6) aged three mouths were divided randomly into model group, growth hormone group(GH) and chinese medicine group, 10 normal accelerated male mice(SAMR1) with the same age were selected as homologous control group. After a week of normal feeding, the mice of GH group and chinese medicine group were given medicine, weighed once a week and we adjusted dose by the weight. The mice of model and normal groups were given equivalent normal saline.Then five mice in each group randomly selected were executed by collecting eyeball blood after 3 mouths and immediately took out the left femur for measuring the bone mass and femoral geometry size and mechanical geometry of bone section and biomechanics capabilities detection. Result Compared with the model group, bone mass decreased in the medication groups after 3 months (P >0.05), and increased after 6 months (P >0.05), and the bone mass of the chinese medicine group was higher than that of the growth hormone group over the same period(P >0.05); 3 months later, cortical bone thickness and sectional area increased (P <0.05), the diameter of the bone marrow cavity and the cross sectional area decreased (P <0.05), the mechanical geometry of bone section increased (P >0.05), the parameters of bone structure mechanics were significantly improved (P <0.01), the maximum stress and elastic stress increased (P <0.01), the stiffness coefficient increased (P <0.05), and the elastic modulus increased (P >0.05) of the SAMP6 in chinese medicine group; 6 months later, the cortical bone thickness and cross-sectional area increased (P <0.01), bone marrow cavity diameter and cross-sectional area decreased (P <0.05), the mechanical geometry of bone section were significantly improved (P <0.01), the maximum load and elastic load increase (P < 0.05), the maximum, maximum bending moment, deflection and energy absorption increased (P >0.05), elastic stress and stiffness coefficient increased (P >0.05), the elastic modulus decreased P >0.05) of the SAMP6 in chinese medicine group; Compared with the growth hormone group, 3 months later, there was no significant difference in the results of each indicator of SAMP6 in chinese medicine group (P >0.05), 6 months later, the cortical bone thickness and cross-sectional area increased significantly (P <0.01), the mechanical geometry of bone section increased (P >0.05), the biomechanical parameters of bone decreased, but there was no statistical significance (P >0.05) in chinese medicine group. Conclusion Bushen Zhuanggu granules could adjust the mechanical response function to macroscopic geometric structure of bone of SAMP6 mice, improve the biomechanical properties of bone, increase its bone toughness and strength, improve its resistance to external forces deformation and fracture; it could increase bone mass in SAMP6 mice with long term administration of Bushen Zhuanggu granules; these results are equivalent to those of SAMP6 mice treated with growth hormone, so there is hope of Bushen Zhuanggu Granules to replace growth hormone, which is expensive and has more side effects, to treat osteoporosis in elderly men.
