补肾活血方调控膝骨性关节炎大鼠软骨下骨的作用机制研究

      目的 探讨补肾活血方对大鼠膝骨性关节炎( knee osteoarthritis,KOA)软骨下骨骨重建中BMP-2及Smad-1/5的调节作用机制。方法 将24只SPF级健康6月龄雌性SD 大鼠随机分为治疗组、模型对照组及假手术组。治疗组、模型对照组参考改良Hulth造模法构建膝骨性关节炎模型,造模成功后4周模型对照组及假手术组灌予生理盐水,治疗组灌予等量补肾活血中药干预,并于灌胃第4、8周,测定软骨下骨BMP-2及Smad-1/5的表达水平。结果 假手术组、治疗组在BV/TV、Tb.N上明显低于模型对照组(P<0.05),在Tb.Th、Tb.Sp上明显高于模型对照组(P<0.05)。假手术组与治疗组Tb.N、Tb.Th、Tb.Sp上比较,差异有统计学意义(P<0.05),用药4周,治疗组BMP-2、Smad-1/5灰度值及阳性细胞个数较模型对照组增加(P<0.05),较假手术组有所减少,差异无统计学意义(P >0.05);用药8周后与用药4周比较,BMP-2、Smad-1/5灰度值及阳性细胞个数增加(P<0.05)。结论 补肾活血方通过提高BMP-2及Smad-1/5在KOA软骨下骨骨平衡分配,改善KOA软骨下骨异常代谢,从而起到治疗KOA的作用。

      Objective To investigate the regulatory mechanism of nourishing kidney and promoting blood flow decoction on BMP-2 and Smad-1/5 in the subchondral bone reconstruction of rats with knee osteoarthritis (KOA). Methods Twenty-four healthy SPF grade 6-month-old female SD rats were randomly divided into treatment group, model control group, and sham operation group. The KOA model was established in the treatment group and the model control group with modified Hulth model. After the model was established, rats in the model control group and the sham operation group received normal saline for 4 weeks, and rats in the treatment group received the same amount of nourishing kidney and promoting blood flow decoction wit gavage. At the 4th and 8th week after intragastric administration, the expression of BMP-2 and Smad-1/5 in subchondral bone was measured. Results BV/TV and Tb.N were significantly lower in the sham operation group and treatment group than those in the model control group (P<0.05), but Tb.Th and Tb.Sp were significantly higher (P<0.05). There was a statistically significant difference between the sham group and the treatment group in Tb.N, Tb.Th, and Tb.Sp (P<0.05). After 4 weeks of treatment, BMP-2 and Smad-1/5 gray values and the number of positive cells in the treatment group increased compared with those in the model control group (P<0.05), but they were lower than in the sham operation group. The difference was not statistically significant (P>0.05). After 8 weeks of treatment, BMP-2, Smad-1/5 gray value, and the number of positive cells increased compared with 4 weeks of treatment (P<0.05). Conclusion Nourishing kidney and promoting blood flow decoction relieves KOA by increasing the distribution of BMP-2 and Smad-1/5 in subchondral bone of KOA and improving the metabolism of subchondral bone in KOA.
