      目的 研究中老年骨质疏松性骨折患者的中医证候特征。方法 纳入2015年10月至2016年1月入住上海龙华医院骨伤科病房的骨质疏松性骨折患者80例,运用跟骨骨密度仪进行足跟部骨密度检测,填写中医证候问卷调查量表,对相关证型数据进行分析和挖掘。结果 中老年骨质疏松性骨折患者中脾肾阳虚型37例,占46.25%;肝肾阴虚型13例,占16.25%;气滞血瘀型14例,占17.50%;其他证型16例,占20.00%。绝经时间较久的妇女中,脾肾阳虚型患者易发生至少2次及以上次数骨折,以髋部、脊柱骨折为主。结论 中老年骨质疏松患者出现骨折多因肌肉协调性下降而摔倒造成,脾主肌肉、肾主骨,若脾肾亏虚则无以濡养肌肉、骨骼,筋缓骨痿,因此,“脾肾相关”“先、后天之本”理论在骨质疏松性骨折防治中具有重要指导作用。

      Objective To study the characteristics of TCM syndromes in elderly patients with osteoporotic fractures. Methods A total of 80 patients with osteoporotic fractures, who were in the orthopedic ward of Longhua Hospital from October 2015 to January 2016, were selected and investigated with questionnaires. Bone mineral density of the heel was measured with calcaneal bone densitometer. The data related with TCM syndromes were analyzed and excavated. Results There were 37 cases of spleen-kidney yang deficiency accounting for 46.25% of the total, 13 cases of liver-kidney yin deficiency accounting for 16.25%, and 14 cases of qi stagnation-blood stasis accounting for 17.50%. The patients with spleen and kidney yang deficiency were prone to fractures at least twice or more times, especially in females with longer period after menopause, who mainly suffered from hip or spine fractures. Conclusion Elderly patients with osteoporosis often suffer from fractures due to falling, causing by decrease of muscle coordination. Spleen governs muscle and kidney governs bone. Muscle-skeletal system becomes weak while spleen and kidney are deficient. Thus the theories of spleen-kidney correlation and congenital and acquired origin play key roles in prevention and treatment of osteoporotic fractures.
