目的 探索老年髋部骨折患者骨膜素(sPostn)水平与骨密度的相关性。方法 纳入108例患有骨质疏松性髋部骨折的老年女性和106名年龄不匹配的无骨折女性作为对照。在2 d内骨折后测量其临床特征、骨密度(bone mineral density,BMD)和骨转换标志物(包括sPostn水平)。对于108例患者,检测1年内的随访sPostn水平。结果 骨折后sPostn的初始水平显著高于对照组。在整个队列中,sPostn与股骨颈BMD,骨Ⅰ型胶原N-端前肽(PINP)和Ⅰ型胶原交联C-末端肽(β-CTX)相关(P均<0.05)。多变量分析后,sPostn仍然是股骨颈BMD的独立危险因素。骨折后7 d内采样的sPostn从第2天开始急剧增加,然后减少并在360 d保持在略高的水平。sPostn的变化与骨折后第7天β-CTX(P<0.05)和PINP(P<0.05)的变化呈正相关。结论 高sPostn水平是老年女性急性髋部骨折患者股骨颈低BMD的独立预测因素。 |

Objective To explore the correlation between periosteal (sPostn) levels and bone mineral density in elderly patients with hip fractures. Methods This study enrolled 108 elderly women with osteoporotic hip fractures and 106 age-matched women without fracture as controls. Clinical features, bone mineral density (BMD), and bone turnover markers including sPostn level were measured in 2 days after fractures. sPostn levels were followed up in 108 patients in 1 year. Results Initial levels of sPostn after fracture were significantly higher in patients than in controls. sPostn was correlated with BMD of the femoral neck (P<0.05), β-isomerized C-terminal crosslinking of type I collagen (β-CTX, P<0.05), and N-terminal procollagen of type I collagen (PINP, P<0.05) in the entire cohort. After multivariate analysis, sPostn remained as an independent risk factor for femoral neck BMD. sPostn acutely increased from day 2 after the fracture sampled within 7 days, and then decreased and maintained at slightly high level in 360 days. The changes of sPostn positively correlated with the variation in β-CTX (P<0.05) and PINP (P<0.05) at day 7 after fracture. Conclusion High sPostn level is an independent predictor of low femoral neck BMD in elder women with acute hip fractures. |
