      目的 观察唑来膦酸对芳香化酶抑制剂辅助治疗乳腺癌患者骨密度的影响。方法 前瞻性评估了102例绝经后临床激素受体阳性乳腺癌I-III A期患者术后12个月内腰椎和双侧股骨颈的骨密度(bone mineral density,BMD)改变。患者术后接受芳香酶抑制剂作为辅助治疗且患有骨质疏松症(腰椎或双侧股骨颈BMD,相当于T值分类≤-2.5)的女性。患者12个月内接受补充钙、维生素D和5 mg静脉注射唑来膦酸治疗。结果 腰椎骨密度在6个月和12个月时分别增加了4.9%和6.6%。右股骨颈和左股骨颈的BMD分别为3.3%和4.1%。在该研究中未观察到低钙血症、颌骨坏死和非创伤性临床骨折。结论 本研究表明,接受佐剂芳香酶抑制剂治疗的妇女接受一次唑来膦酸治疗后BMD大幅增加,无论其芳香酶抑制剂治疗的历史如何。

      Objective To observe the effect of zoledronic acid on the bone mineral density in breast cancer patients treated with aromatase inhibitors. Methods We prospectively evaluated the bone mineral density (BMD) in the lumbar spine and the bilateral femoral necks in 102 postmenopausal women with stage I–IIIA clinical hormone receptor-positive breast cancer, during a postoperative period of 12months. The included patients received aromatase inhibitors as adjuvant therapy and with evidence of osteoporosis (lumbar spine or bilateral femoral neck BMD, equivalent to T-score classification of?≤-2.5) upon enrollment. The patients received supplemental calcium, vitamin D, and 5mg of intravenous injection zoledronic acid every 12 months. Results BMD of the lumbar spine increased by 4.9 and 6.6% at 6 and 12months, respectively. BMD of the right and left femoral neck increased by 3.3% and 4.1% at 12months, respectively. Hypocalcemia, osteonecrosis of the jaw, and non-traumatic clinical fracture were not observed in this study. Conclusion Our findings reveal that treatment with zoledronic acid once is associated with a marked increase of BMD in women receiving adjuvant aromatase inhibitor therapy, which is irrespective of their previous history of aromatase inhibitor therapy.
