| 目的 探讨Charlson合并症指数（charlson complication index，CCI）评分对于脆性髋部骨折患者死亡率的影响。方法 通过回顾性分析2010年1月至2015年12月在我院住院治疗并获得有效随访的老年脆性髋部骨折患者的资料，从住院病历资料中对患者进行回顾性CCI评分，分别计算出每个患者的CCI总评分（合并症评分+年龄评分）和合并症评分，随访患者的存活情况，统计分析CCI评分与患者1年死亡率、随访期死亡率的关系。结果 2010年1月至2015年12月在我院住院治疗，年龄超过50岁的并获得有效随访的老年脆性髋部骨折患者共690例，平均年龄（77.19±10.38）岁，患者随访时间为7～52个月，平均（28.53±9.75）个月。在这些患者中，1年内累计死亡99例，死亡率为15.60%（99/635）；随访期总共累计死亡166例，随访期总死亡率为24.06%（166/690）。根据CCI总评分分成两组（＜5分和≥5分），＜5分组的患者1年死亡率、随访期总死亡率更低，两组比较差异有统计学意义（P＜0.05）。根据CCI合并症评分分成两组（＜1分和≥1分），＜1分组患者1年死亡率、随访期总死亡率更低，两组比较差异有统计学意义（P＜0.05）。结论 不同的CCI评分对老年脆性髋部骨折的1年死亡率、随访期总死亡率有一定的影响，住院时对患者进行CCI评分为老年脆性髋部骨折患者提供良好的术前指标，有助预测治疗结果，指导临床治疗。 |

| Objective The aim of this study was to investigate the effect of Charlson complication index (CCI) score on the mortality of patients with fragile hip fractures. Methods The data of elderly patients with fragile hip fractures treated in our hospital from January 2010 to December 2015 were retrospectively analyzed. The CCI score was retrospectively analyzed from the medical records. The total CCI score (complication score + age score) and complication score of each patient were calculated respectively. The relationship between CCI score and 1-year mortality and follow-up mortality was analyzed. Results From January 2010 to December 2015, there were 690 over 50-year-old elderly patients with fragile hip fractures received effective follow-up in our hospital. The average age of the patients was 77.19 ± 10.38 years old. The patients were followed up for 7 to 52 months, with an average follow-up of 28.53 ± 9.75 months. Among these patients, 99 died in one year, with a mortality rate of 15.60% (99/635), and 166 died during the follow-up period, with a total mortality rate of 24.06% (166/690). According to the CCI total score, the patients were divided into two groups (5 score and ≥5 score). The one-year mortality rate and the total mortality during follow-up period were lower in the group of less than 5 score. There was significant statistical difference between the two groups (P<0.05). According to CCI complication score, the patients were divided into two groups (<1 score and ≥1 score). The one-year mortality rate and the total mortality during follow-up period were lower in the group of less than 1 score. There was significant statistical difference between the two groups (P<0.05). Conclusion Different CCI scores have certain effect on the one-year mortality and the total mortality during follow-up period of fragile hip fractures in the elderly. CCI score provides good preoperative indicators for elderly patients with fragile hip fracture, which helps to predict the outcome of treatment and to guide our treatment. |
