      目的 探讨血清1-磷酸鞘氨醇(S1P)与绝经后2型糖尿病患者骨密度(bone mineral density,BMD) 和骨代谢指标之间的相关性。方法 选取2018年2月至2019年12月期间在海口市妇幼保健院就诊的绝经后2型糖尿病女性,收集患者一般临床资料和获取其血液标本,检测生化指标、S1P和髋部、腰椎骨密度。结果 最终选取130名血糖控制较好的绝经后2型糖尿病女性参与本研究,年龄为(59.3±8.9)岁,血糖为(8.75±1.5)mmol/L;S1P平均浓度为(6.46±0.78)μmol/L。相关分析表明S1P与腰椎(L1~4)、全髋和股骨颈BMD呈显著负相关(P均<0.05)。多步逐步回归分析表明,血清S1P和Ⅰ型胶原交联C末端肽(β-CTX)与腰椎(L1~4)、全髋和股骨颈BMD密切相关;而血清S1P和β-CTX是各部位BMD独立危险因素。结论 1-磷酸鞘氨醇与绝经后2型糖尿病女性骨密度和β-CTX水平密切相关。

      Objective To explore the correlation between Sphingosine 1-phosphate (S1P) and bone metabolism index and bone mineral density (BMD) in postmenopausal patients with type 2 diabetes. Methods Women with postmenopausal type 2 diabetes who visited our hospital from February 2018 to December 2019 were selected to collect general clinical data and obtain blood samples from patients. Biochemical indicators were measured, as well as S1P and hip and lumbar bone density. Results Finally, 130 postmenopausal women with type 2 diabetes with good blood glucose control were selected to participate in this study. The age was (59.3 ±8.9) years old. The average concentration of blood glucose was (8.75 ±1.5) mmol/L and the average S1P was (6.46 ±0.78) μmol/L. S1P was negatively correlated with BMD of lumbar vertebrae (L1-4), total hip and femoral neck (all P<0.05). Multi-step stepwise regression analysis showed that serum S1P and type I collagen cross-linked C-terminal peptide (β-CTX) were closely related to lumbar vertebra (L1-4), total hip and femoral neck BMD, while serum S1P and β-CTX were the independent risk factors for BMD. Conclusion Sphingosine 1-phosphate is closely related to bone mineral density and β-CTX levels in postmenopausal women with type 2 diabetes.
