| 目的 应用倾向性评分匹配回顾性分析锝[99Tc]亚甲基双膦酸盐注射液（云克）对长病程类风湿关节炎患者骨折和跌倒发生率的影响。方法 选取病程超过10年类风湿关节炎患者118例，其中未应用云克的类风湿关节炎患者63例作为对照组，应用云克的类风湿关节炎患者55例作为治疗组。应用倾向性评分匹配以均衡组间协变量，比较两组1年后骨折和跌倒发生率。结果 两组间FRAX主要部位骨折评分和FRAX髋部骨折评分差异具有统计学意义（均P＜0.05）。经倾向性评分匹配，选用最近邻匹配法，设置卡钳值为0.2，按照对照组与治疗组1∶1进行匹配，成功匹配对照组和治疗组各44例。治疗组骨折发生率低于对照组（3例vs 10例，P＜0.05），跌倒发生率低于对照组（3例 vs 11例，P＜0.05）。结论 云克治疗有助于减少类风湿关节炎骨折和跌倒发生率。 |

| Objective To retrospectively analyze the effects of Yunke on the incidence of fracture and fall in patients with long-course rheumatoid arthritis (RA) in the real world with propensity score matching. Methods A total of 118 patients with rheumatoid arthritis whose course of disease was over 10 years were selected, among them, 63 patients with rheumatoid arthritis who had not been treated with Yunke were selected as the control group and 55 patients who had been treated with Yunke were selected as the treatment group. The incidence of fractures and falls after 1 year was compared between the two groups by propensity score matching. Results There were statistically significant differences between the FRAX scores for major osteoporotic fracture and the FRAX scores for hip fracture between the two groups (both P<0.05). After propensity score matching, the nearest neighbor matching method was used, and the caliper value was set as 0.2. The control group and the treatment group were matched 1:1, and 44 cases were successfully matched in each group. The incidence of fracture in the treatment group was lower than that in the control group (3 cases vs 10 cases, P < 0.05), and the incidence of fall was lower than that in the control group (3 cases vs 11 cases, P < 0.05). Conclusion Yunke therapy is helpful to reduce the incidence of fracture and fall in rheumatoid arthritis. |
