目的 探讨绝经后女性骨密度与腰椎间盘退变程度的相关性。方法 回顾性研究西南医科大学附属医院2017年 1月至2019年12月收治的因慢性腰痛住院的251例绝经后女性患者。收集患者的年龄、绝经年限、体质量指数(body mass index,BMI)。采用双能X线骨密度仪检测患者腰椎(L1~4)及髋部平均骨密度,根据骨密度T值将患者分为骨量正常组(n = 45)、骨量低下组(n = 71)和骨质疏松组(n =135)。同时患者均行腰椎MRI检査,采用Pfirrmann分级系统评估腰椎间盘退变 程度(L1/2、L2/3、L3/4、L4/5、L5/S1)。采用Spearman相关性分析骨密度与腰椎间盘退变程度的关系。结果 骨质疏松组和骨量低下组的年龄、绝经年限显著大于骨量正常组(P<0. 05)。骨质疏松组和骨量低下组的BMI明显小于骨量正常组(P< 0.05)。骨质疏松组和骨量低下组的L1/2、L2/3、L3/4、L1~S1(L1/2~L5/S1的平均值)椎间盘退变评分明显髙于骨量正常组 (P<0. 05)。骨质疏松组的L4/5、L5/S1椎间盘退变评分髙于骨量正常组(P<0. 05),骨量低下组与骨量正常组比较差异无统计学意义(P>0.05)。相关性分析结果显示,骨密度与BMI呈正相关(P<0.05),与年龄、绝经年限、Ll/2、L2/3、L3/4、L4/5、 L5/S1、L1~S1椎间盘退变评分呈负相关(P<0. 05)。结论 绝经后女性骨密度与BMI呈正相关,与年龄、绝经年限、腰椎间盘 退变程度呈负相关。绝经后女性骨密度越低,其腰椎退变程度越严重。 |

Objective To explore the correlation between the bone mineral density and the degree of lumbar disc degeneration in postmenopausal women. Methods A retrospective study of 251 postmenopausal female patients hospitalized with chronic low back pain from January 2017 to December 2019 in our hospital. Collected the patient's age,years of menopause,and body mass index (BMI). The dual-energy X-ray bone densitometer was used to detect the average bone mineral density of the patient's lumbar spine (L1-4) and hip,and the patients were divided into three groups according to the bone mineral density T value,normal bone mass group (n = 45),bone mass reduction group (n = 71) and osteoporosis group (n = 135). At the same time,all patients underwent MRI of the lumbar spine,and the Pfirrmann grading system was used to evaluate the degree of lumbar disc degeneration (L1/2, L2/3,L3/4,L4/5,L5/S1). Used Spearman correlation to analyze the relationship between bone mineral density and the degree of lumbar disc degeneration. Results The age and menopausal years of osteoporosis group and bone mass reduction group were significantly greater than those of normal bone mass group (P <0.05). The BMI of osteoporosis group and bone mass reduction group was significantly lower than that of normal bone mass group (P <0.05). The scores of L1/2,L2/3,L3/4,L1-S1 intervertebral disc degeneration in osteoporosis group and bone mass reduction group were significantly higher than those in normal bone mass group (P<0.05). The osteoporosis group's L4/5,L5/S1 intervertebral disc degeneration scores were higher than those with normal bone mass (P <0. 05),and there was no significant difference between the bone loss group and the normal bone mass group (P> 0. 05). Correlation analysis result showed that bone mineral density was positively correlated with BMI (P<0. 05),and negatively correlated with age,menopause years,L1/2,L2/3,L3/4,L4/5,L5/S1,L1-S1 disc degeneration score (P<0. 05).Conclusion Postmenopausal women ’ s bone mineral density was positively correlated with BMI, but negatively correlated with age, menopausal age, and degree of lumbar disc degeneration. The lower the bone density of postmenopausal women, the more severe the degree of lumbar degeneration. |
