目的 采用静息态功能磁共振(RS-fMRI)观察雷火灸干预原发性骨质疏松症腰背疼痛患者自发性脑活动的影响,并分析自发性脑活动与疼痛改善的相关性。方法 入组原发性骨质疏松症腰背痛患者18例,给予雷火灸治疗,取穴肝俞、肾俞、大肠俞、命门、腰阳关,每组腧穴治疗30 min,每周3次,隔日1次,持续4周。治疗4周前后采用视觉模拟评分(VAS)和RS-fMRI进行评估。结果 ①VAS治疗后较治疗前显著改善(P<0.05);②与治疗前比较,治疗后以左侧岛叶、左侧颞下回、左侧海马为代表的脑区低频振幅(ALFF)值显著降低,且ALFF值与VAS评分差值具有显著正相关。结论 雷火灸治疗能缓解原发性骨质疏松症(POP)患者慢性腰背疼痛,其调节自发性脑活动可能是重要的作用机制之一。 |

Objective Resting functional magnetic resonance (RS-fMRI) was used to observe the effect of thunder-fire moxibustion on spontaneous brain activity in primary osteoporosis patients with low back pain, and to analyze the correlation between spontaneous brain activity and pain improvement. Methods Eighteen patients with primary osteoporosis and low back pain were enrolled. They were treated with thunder-fire moxibustion. The acupoints of Ganshu (BL 18), Shenshu (BL 23), Dachangshu (BL 25), Mingmen (DU 4), and Yaoyangguan (DU 3) were selected. The treatment lasted for 30 minutes, with 3 times a week, once every other day, for 4 weeks. Visual analogue scale (VAS) and RS-fMRI were used for evaluation before and after 4 weeks of the treatment. Results ①VAS was significantly improved after the treatment than before (P<0.05). ②Compared to those before the treatment, the brain area low frequency amplitude (ALFF) value represented by the left insula, left inferior temporal gyrus, and left hippocampus decreased significantly after the treatment, and the ALFF value had a significant positive correlation with the VAS score difference. Conclusion Thunder-fire moxibustion can relieve chronic back pain in primary osteoporosis (POP) patients, and its regulation of spontaneous brain activity maybe one of the important mechanisms. |
