      目的 探讨老年营养风险指数(geriatric nutritional risk index,GNRI)与老年男性2型糖尿病患者骨密度(bone mineral density, BMD)和骨质疏松症的关系。方法 选取2019年1月至2019年8月在长沙市第一医院内分泌科住院的老年男性2型糖尿病患者216例,根据GNRI值分为降低组(GNRI≤98)和正常组(GNRI>98),收集患者的一般临床资料,分析GNRI与老年男性2型糖尿病患者骨密度和骨质疏松症的关系。结果 ①与GNRI正常组比较,GNRI降低组股骨颈、全髋BMD更低(P<0.01),骨质疏松症患病率更高(P<0.05);②GNRI与股骨颈BMD呈正相关(P<0.01)、与全髋BMD呈正相关(P<0.05);③多元Logistic回归分析GNRI降低是骨质疏松症的独立危险因素(B= – 0.076,P<0.05)。ROC曲线显示包含GNRI的联合指标对老年男性2型糖尿病患者骨质疏松症的预测价值较高。结论 老年男性2型糖尿病患者GNRI与股骨颈、全髋BMD呈正相关,GNRI降低是骨质疏松症的独立危险因素。

      Objective To investigate the association between the Geriatric Nutritional Risk Index, bone mineral density and osteoporosis in elderly male patients with type 2 diabetes mellitus. Methods 216 elderly men with type 2 diabetes mellitus in hospital from January 2019 to August 2019 were selected and divided into the reduced GNRI group (GNRI≤98) and the normal group (GNRI > 98). Clinical data of patients were collected to analyze the relationship between GNRI and bone mineral density and osteoporosis in elderly male patients with type 2 diabetes. Results ① Compared with the normal GNRI group, the reduced GNRI group had a lower femoral neck and total hip BMD(P<0.01),and a higher incidence of osteoporosis(P<0.05).② GNRI was positively correlated with femoral neck BMD (P<0.01) and total hip BMD (P<0.05). ③Multiple logistic regression analysis showed that reduced GNRI was an independent risk factor for osteoporosis (B= – 0.076,P<0.05). The ROC curve showed that the combined indexes including GNRI had a higher value in predicting osteoporosis in elderly male patients with type 2 diabetes mellitus. Conclusion GNRI was positively correlated with BMD of femoral neck and total hip in elderly male patients with type 2 diabetes mellitus, and the reduced GNRI was an independent risk factor for osteoporosis.
