      骨性关节炎是一种全世界范围内发病率高的慢性退行性疾病,其病理特征为关节软骨退化和软骨下骨结构改变并伴有滑膜炎症和骨赘形成。虽然该病的研究取得了一些进展成果,但其确切的病因及发病机制仍不完全清楚。滑膜巨噬细胞作为关节滑膜中重要的炎症细胞,其不同的极化状态在骨性关节炎的发病中起着重要作用。巨噬细胞受其所处不同微环境的刺激时极化为具有促炎作用的M1型巨噬细胞和抗炎作用的M2型巨噬细胞,促使未极化的巨噬细胞向M2型极化或调控M1型巨噬细胞向M2型转化,对抑制骨性关节炎的发展起到积极作用。笔者通过查阅近几年相关研究文献,就滑膜巨噬细胞在骨性关节炎发病机制中的作用及其潜在性应用展开综述,为后期骨性关节炎的诊治及研究提供新的思路和方向。

      Osteoarthritis (OA) is a chronic degenerative disease with a high incidence worldwide, which is pathologically characterized by articular cartilage degeneration and subchondral bone structure changes accompanied by synovitis and osteophyte formation. Although some progress has been made in the study of this disease, its exact etiology and pathogenesis are still not completely clear. As an important inflammatory cell in synovial membrane, synovial macrophages in different polarization states play an important role in the pathogenesis of osteoarthritis.When stimulated by different microenvironments, macrophages are polarized into M1-type macrophages with pro-inflammatory effect and M2-type macrophages with anti-inflammatory effect, which can promote the polarization of unpolarized macrophages to M2-type macrophages or regulate the transformation of M1-type macrophages to M2-type macrophages, and play a positive role in inhibiting the development of osteoarthritis. This paper reviews the role and potential application of synovial macrophages in the pathogenesis of osteoarthritis by referring to relevant research literatures in recent years, so as to provide new ideas and directions for the diagnosis, treatment and research of osteoarthritis in the later stage.
