目的 通过分析非同步校准QCT测量腰椎骨密度在较长时间间隔内的精密度误差( precision error,PE),评估其长期体内可重复性。方法 收集2018年6月至2019年12月于贵州医科大学附属医院进行2次非同步QCT腰椎骨密度检查的健康体检者(共526名)并按国际临床骨密度学会(ISCD)骨质疏松诊断标准分为3组(骨质疏松组、低骨量组、骨量正常组),2次检查间隔时间约1年,由同1名接受培训后的影像科住院医师测量受试者腰椎骨密度,计算其精密度误差。采用非参数检验进行组间比较。结果 非同步QCT测量腰椎骨密度的总体精密度误差为:RMS-% CV=2.57 %,RMS-SD=3.14 mg/cm3;不同组间变异系数(coefficient of variation,CV)差异有统计学意义(P<0.05)。结论 非同步校准QCT测量腰椎骨密度的长期体内可重复性好,精密度高,可用于临床随访观察;低骨量人群的腰椎骨密度变异度较骨量正常的人群高,骨密度较高的人群具有更好的可重复性。 |

Objective To evaluate the long-term in vivo reproducibility by calculating the precision error of asynchronous calibration QCT measurement of lumbar spine bone mineral density (BMD) over a long time interval. Methods Collected the population receiving physical examinations (526 people in total) who underwent two asynchronous calibration QCT lumbar spine bone density examinations in Affiliated Hospital of Guizhou Medical University from June 2018 to December 2019, and divided them into 3 groups according to ISCD osteoporosis diagnostic criteria (osteoporosis, low bone mass, normal bone mass), the interval between two examinations was about 1 year, the lumbar spine BMD was measured by the same technician, and the precision error was calculated. Results The overall precision error of the asynchronous calibration QCT measurement of lumbar spine bone density was: RMS-% CV=2.57 %, RMS-SD=3.14 mg/cm3; the difference in coefficient of variation between different groups was statistically significant (P<0.05). Conclusion Asynchronous calibration QCT has good long-term in vivo reproducibility and high precision in measuring lumbar spine bone mineral density, and can be used for clinical follow-up observation; people with low bone density have higher variability, and people with higher bone density have greater reproducibility. |
