      目的 通过定量CT(quantitative computed tomography,QCT)研究中老年体检女性血常规指标与腰椎骨密度的相关性问题。方法 选取2021年10月至12月在皖南医学院第一附属医院健康管理中心体检的中老年女性1 000名,收集其血常规指标、血糖、血脂、年龄、体质量指数(body mass index, BMI),同时使用QCT测量受试者腰椎骨密度(bone mineral density,BMD)。根据骨密度值分为骨量正常组(≥120 mg/cm3)、骨量减少组(80~120 mg/cm3)和骨质疏松组(≤80 mg/cm3)。采用单因素方差分析不同组间血常规指标、血糖、血脂的差异。利用Pearson相关性分析血小板计数与骨密度的相关性。结果 不同BMD组间年龄、血小板计数、空腹血糖比较差异具有统计学意义(P<0.01)。Pearson相关分析显示,血小板计数与L1 BMD、L2 BMD、L1~2平均骨密度(M-BMD)呈正相关(r=0.155、0.113、0.165,P<0.01),差异具有统计学意义。校正年龄、BMI、空腹血糖后,偏相关分析显示血小板计数与M-BMD仍呈一定正相关(r=0.153,P<0.01)。结论 血小板计数与中老年女性腰椎骨密度具有一定相关性,对骨质疏松症的早期防治、预警具有一定临床价值。

      Objective To explore the correlation between blood routine index and lumbar bone mineral density in middle-aged and elderly women based on quantitative CT. Methods A total of 1 000 physical examinations were performed in Health Management Center of the First Affiliated Hospital of Wannan Medical College from October to December 2021. Routine blood indexes, blood glucose, blood lipids, age, BMI were collected through the computer system and lumbar bone mineral densitometry by quantitative CT. According to the results of bone mineral density, the subjects were divided into normal bone mass group (≥120 mg/cm3), reduced bone mass group (80-120 mg/cm3) and osteoporosis group (≤80 mg/cm3). Univariate ANOVA was used to analyze the differences of blood routine indexes, blood glucose and blood lipids among different groups. The correlation between platelet count and bone mineral density was analyzed using pearson correlation. Results There were significant differences in age, platelet count and fasting blood glucose among different BMD groups (P<0.01). Pearson correlation analysis showed that platelet count was positively correlated with L1 BMD, L2 BMD and L1-2 mean bone mineral density(M-BMD) (r=0.155, 0.113, 0.165, P<0.01), with statistically significant differences. Partial correlation analysis showed a positive correlation between platelet count and M-BMD after adjusting for age, BMI and fasting blood glucose (r =0.153, P<0.01). Conclusion Platelet count has certain correlation with lumbar vertebra bone density in middle-aged and elderly women, which has certain value for early prevention and early warning of osteoporosis.
