目的 探讨2型糖尿病(type 2 diabetes mellitus, T2DM)患者微血管病变(micro vascular disease, MVD)与骨密度(bone mineral density, BMD)的相关性。方法 横断面研究广西医科大学第一附属医院收治的317例T2DM住院患者(男性均≥50岁,女性均已绝经)相关资料,按是否合并糖尿病微血管病变将患者分为MVD组(178例)、无MVD组(139例)。运用多因素Logistic 回归分析评估微血管病变与骨密度的相关性。结果 T2DM合并MVD者的骨质疏松症患病率较无MVD者更高(43% vs 23%,P=0.007)。在调整了糖尿病病程、性别、年龄、体质量指数(body mass index, BMI)、高血压、骨钙素、甲状旁腺素后,多因素回归显示女性、年龄≥65岁、合并高血压是T2DM合并骨质疏松症的独立危险因素,而BMI是独立保护因素。进一步分层分析显示在调整混杂因素后,男性T2DM患者合并微血管病变是骨质疏松症的独立危险因素,而在女性患者中无相关关系。结论 男性T2DM患者合并微血管病变是骨质疏松症的独立危险因素。 |

Objective To investigate the correlation between microangiopathy and bone mineral density (BMD) in patients with type 2 diabetes (T2DM). Methods A cross-sectional study was conducted to collect relevant data from 317 inpatients with T2DM (men are 50 or more years of age, female were menopausal) in the First Affiliated Hospital of Guangxi Medical University. The two groups were formed from diabetic patients by microangiopathy: 178 cases in the MVD(micro vascular desease)group; 139 cases in the non-MVD group. The correlation between microangiopathy and BMD was evaluated by multivariate analysis. Results The prevalence of osteoporosis in T2DM patients with MVD was higher than that without MVD (43% vs 23% , P=0.007). After adjusting for duration of diabetes, sex, age, BMI, hypertension, osteocalcin, and parathyroid hormone, multivariate analysis showed that female, older than 65 years old, and hypertension were independent risk factors of osteoporosis with T2DM, whereas BMI was an independent protective factor. Stratification analysis showed that microangiopathy was an independent risk factor of osteoporosis in male T2DM patients after adjusting for confounders, but not in female. Conclusion Microangiopathy is an independent risk factor of osteoporosis in male patients with T2DM. |
