      骨质疏松症(osteoporosis,OP)是一种全身性代谢性骨骼疾病,目前公认的发病机制主要为骨吸收大于骨形成所引起的骨重塑失衡。微小核糖核酸(MicroRNA,miRNA)在参与调控骨重塑的基因表观遗传机制中扮演重要角色,可通过靶向上游关键调控因子(Runx2、RANKL等)及相关信号通路(Wnt/β-catenin通路、PI3K/Akt通路等)调控骨髓间充质干细胞成骨分化、成骨细胞分化及破骨细胞分化等细胞代谢进而影响骨重塑。中医药防治OP历史悠久,确有疗效,具有多通路、多靶点协同的特点。随着分子生物学研究的不断深入,中医药可通过干预多种miRNA表达参与调控成骨及破骨分化,进而改善骨代谢防治OP。该文将通过总结多种miRNA调控相关因子影响成骨及破骨分化的靶向作用机制及中医药干预miRNA防治OP的研究成果,为探索miRNA在OP中的作用机制及中医药防治OP提供理论依据。

      Osteoporosis (OP) is a kind of systemic metabolic bone disease. Its pathogenesis is generally recognized to be the imbalance of bone remodeling caused by bone resorption rather than bone formation. MicroRNA (miRNA) plays an important role in the epigenetic mechanism involved in regulating bone remodeling. By targeting upstream key regulatory factors (Runx2, RANKL, etc.) and related signaling pathways (Wnt/β-catenin pathway, PI3K/Akt pathway, etc.), bone remodeling is influenced by regulation of osteogenic differentiation, osteoblast differentiation, and osteoclast differentiation by bone marrow mesenchymal stem cells. Traditional Chinese medicine (TCM) has a long history in the prevention and treatment of OP. It is effective and has the characteristics of multi-pathway and multi-target coordination. With the deepening of molecular biology research, traditional Chinese medicine is involved in the regulation of osteogenesis and osteoclast differentiation by interfering with the expression of multiple miRNA, so as to improve bone metabolism and to prevent OP. This paper summarizes the targeted mechanism of action of various miRNA regulatory factors affecting osteogenesis and osteoclast differentiation and the research results of TCM intervention in the prevention and treatment of OP by miRNA, so as to provide theoretical basis for exploring the mechanism of miRNA in OP and TCM prevention and treatment of OP.
