| 目的 分析江西地区不同等级医院、不同职称的骨科医师对骨质疏松性椎体骨折（osteoporotic vertebral compression fracture，OVCF）的诊疗和认知现状。方法 采用横断面研究，使用“问卷星”平台设计问卷通过微信发放。调查对象为江西地区具有职业资格的骨科医师，问卷的内容包括：基本信息，OVCF诊断、治疗、管理的选择以及学习现状。结果 共回收279份有效问卷，其中初级职称67人、中级职称106人、高级职称106人。诊断方面：三甲医院骨科医师选择QCT诊断骨质疏松症的比例更高（P＜0.05）。不同等级医院、不同职称的骨科医师对骨质疏松症检查对象的选择存在差异（P＜0.05）；治疗方面：中高级职称医师倾向于手术治疗OVCF（P＜0.05）。不同等级医院、不同职称的骨科医师对于抗骨质疏松症药物的选择差异大（P＜0.05）；管理方面：初级职称医师有抗骨质疏松症治疗方案的比例更低（P＜0.05），调查对象建立随访比例较低；学习方面：初、中级职称医师系统的骨质疏松症专业知识学习比例低（P＜0.05）。结论 江西地区骨科医师对OVCF的诊疗选择因医院等级、职称不同，差异性较大。三甲医院医师倾向于选择QCT诊断骨质疏松症；中高级职称和三级医院医师倾向于选择手术治疗OVCF；大多数调查对象缺乏对OVCF的管理方案；初、中级职称骨科医师缺乏系统专业地学习骨质疏松症相关知识。 |

| Objective To analyze the current status of diagnosis, treatment and cognition of osteoporotic vertebral compression fracture(OVCF) among orthopedic surgeons with different hospital grades and different professional titlesin Jiangxi province. Methods A cross-sectional study was used.we adopted the "Questionnaire Star" network system to design the questionnaire and distributed QR code through the WeChat app. Participants were orthopedic surgeons with professional qualifications in Jiangxi province.The contents of the questionnaire including basicinformation, diagnosis, treatment, management and learning status of the respondents of OVCF. Results A total of 279 questionnaires were returned: 67 respondents carried primary titles,106 respondents carried intermediate titles and 106 respondents carried senior titles.With respect to diagnosis: A higher proportion of respondents from Grade III-A hospitals opted for QCT to detect bone mineral density (P＜0.05).There are differences in the selection of osteoporosis examination objects among orthopedic surgeons with different hospital grades and different professional titles (P＜0.05). With respect to treatment: Surgeons with intermediate titles or senior professional titles tended to choose surgery for OVCF(P＜0.05). There were significant differences in the choice of anti-osteoporosis drugs among orthopedic surgeons with different hospital grades and different professional titles (P＜0.05).With respect to management: Orthopedic surgeons with primary titles had less treatment plans for anti-osteoporosis treatment (P＜0.05).The overall proportion of follow-up was low. With respect to learning status: The rate of osteoporosis professional knowledge learning in junior and intermediate orthopedic surgeons was lower than that in senior orthopedic doctors (P< 0.05). Conclusion The selection of diagnosis and treatment of OVCF by orthopedic surgeons in Jiangxi province varies greatly due to different hospital grades and different professional titles. Orthopedic surgeons in Grade III-A hospitals tended to choose QCT to diagnose osteoporosis. Orthopedic surgeons with intermediate titles or senior titles tended to choose surgery for OVCF. Most of the orthopedic surgeons who participated in the survey lacked an OVCF management plan. Orthopedic surgeons with primary and intermediate titles lack systematic professional knowledge learning of osteoporosis. |
