      膝关节骨性关节炎(knee osteoarthritis,KOA)是一种慢性退行性疾病,疼痛是其最常见和最主要的症状。有研究表明,KOA疼痛的产生可能与炎症因子释放、软骨细胞凋亡有关。神经生长因子(nerve growth factor, NGF)/肌钙蛋白受体激酶A(tropomyosin receptor kinase A,TrkA)、核因子E2相关因子2(nuclear transcription factor E2 related factor 2, Nrf2)/血红素加氧酶-1(heme oxygenase 1, HO-1)、p38丝裂原活化蛋白激酶(p38 mitogen-activated protein kinase, p38 MAPK)、磷酸酰肌醇3-激酶(phosphatidylinositol-3-ki-ases, PI3K)/蛋白质激酶B(protein kinase B, Akt)等信号通路均可通过减少炎症因子释放、减轻炎症反应、抑制软骨细胞凋亡,达到改善KOA疼痛的作用。中医药治疗“痹症”、“膝痹”疗效显著,中药单体、中药复方均可通过上述信号通路发挥减轻KOA疼痛的作用。兹本文通过总结上述信号通路与中药单体及复方干预的研究现状,旨为中医药防治KOA研究提供新思路,为中医药临床改善KOA疼痛提供理论参考与依据。

      Knee osteoarthritis (KOA) is a chronic degenerative disease in which pain is the most common and major symptom. Studies have shown that the production of KOA pain may be related to the release of inflammatory cytokines and the apoptosis of chondrocytes. Nerve growth factor (NGF)/tropomyosin receptor kinase A (TrkA), nuclear transcription factor E2 related factor 2 (Nrf2)/heme oxygenase 1 (HO-1), p38 mitogen activated protein kinase (p38 MAPK), and phosphatidylinositol-3-kinases (PI3K)/protein kinase B (Akt) signaling pathways relieve KOA pain by reducing the release of inflammatory cytokines, reducing inflammatory response, and inhibiting chondrocyte apoptosis. Traditional Chinese medicine (TCM) is effective in treating "arthralgia" and "knee arthralgia". Both TCM monomeric and compound relieve the pain of KOA through the above signaling pathways. By summarizing the above signaling pathways and the status of intervention studies on TCM monomer and compound, this paper aims to provide new ideas for TCM prevention and treatment of KOA, and to provide theoretical reference and basis for relieving KOA pain with TCM.
