健脾利湿药治疗骨质疏松症的脂代谢调控相关机制探讨

      骨质疏松症(osteoporosis,OP)是以骨量减少、骨微结构退化为特征的一种中老年常见病,该病患者在发病过程中常伴有肥胖、高脂血症、动脉粥样硬化等脂代谢异常相关疾病。中医理论认为,OP与“骨痿”“骨痹”的症状相似,脂代谢异常与“膏脂化生异常”症状相似,“脾虚湿困”是二者的共同病机。本文分析了脂肪组织及脂代谢影响骨质疏松的途径,并对健脾利湿类中药治疗OP的最新研究进展进行总结。分析发现,对脂代谢异常具有显著改善作用的健脾利湿药在治疗骨质疏松症方面也有着巨大潜力,其可通过多种途径影响骨质疏松的发生发展,其中包括抑制间充质干细胞成脂分化、促进成骨细胞增殖、抑制破骨细胞的活性、调节骨代谢相关因子等。并且有临床研究也证实了健脾利湿药在骨质疏松症治疗中的有效性。综上所述,健脾利湿药在发挥调节脂代谢作用的同时,也是治疗OP的潜在药物,其基于脂代谢发挥骨保护作用的机制可能成为新的研究热点。

      Osteoporosis is a common disease in middle-aged and elderly people characterized by a decrease in bone mass and degradation of bone microstructure, often accompanied by lipid metabolism-related diseases such as obesity, hyperlipidemia, and atherosclerosis during the onset process. According to traditional Chinese medicine theory, the symptoms of osteoporosis are similar to those of "bone wilting" and "bone impediment", and lipid metabolism disorders are similar to the symptoms of "Gao and Zhi transformation abnormality", while "spleen deficiency with dampness encumbrance" is the common pathogenesis of the two. This article analyzes the pathways through which adipose tissue and lipid metabolism affect osteoporosis, and summarizes the latest research progress on the use of the traditional Chinese medicine for fortifying the spleen and draining dampness in the treatment of osteoporosis. The analysis shows that spleen-fortifying and dampness-draining medicine not only have significant regulation effect on lipid metabolism, but also have great potential in the treatment of osteoporosis. They can prevent and slow down osteoporosis through multiple pathways, including inhibiting the adipogenic differentiation of mesenchymal stem cells, promoting osteoblast proliferation, inhibiting osteoclast activity, and regulating bone metabolism-related factors. In addition, clinical studies have confirmed the effectiveness of spleen-fortifying and dampness-draining medicine in the treatment of osteoporosis. Therefore, spleen-fortifying and dampness-draining medicine, while playing a role in regulating lipid metabolism, is also a potential medicine for the treatment of osteoporosis. Its mechanism based on lipid metabolism exerting a bone-protective effect may become a new research hotspot.
