      脾乃后天之本,脾脏运化功能正常是气血生化有源的前提,亦是精微物质得以输布周身的必要条件。线粒体是能量代谢的主要场所,而脾生气血以推动机体各类运动的作用与线粒体为机体活动提供能量的功能相似。线粒体自噬(mitophagy)主要通过清除损伤的线粒体以维持细胞稳态,其发生与动力相关蛋白1(DRP1)的活性密切相关,而脾作为气血生化之源对维持机体所需动力极其重要,相关研究发现健脾可以增加Parkin的表达进而促进线粒体自噬。骨质疏松是糖尿病的常见并发症之一,其发生与胰岛素抵抗所致肌肉含量下降、血脂代谢紊乱及高糖环境下线粒体因过多产生的氧化自由基诱导的氧化应激发生功能退化导致的微血管病变、动脉粥样硬化密切相关,而线粒体自噬可以通过清除损伤的线粒体,减少其对结构功能健全的线粒体的损伤以降低胰岛素抵抗程度和微血管病变、动脉粥样硬化发生的风险,从而防治糖尿病性骨质疏松。故该文基于“脾-线粒体”相关理论探讨线粒体自噬与糖尿病性骨质疏松的联系。

      Spleen is the acquired essence. Normal transport function of spleen is the premise of Qi and blood biochemistry. Is also a necessary condition for fine substances to be transported throughout the body. Mitochondria are the main sites of energy metabolism and the function of spleen to generate Qi and blood to promote various kinds of body movements. It is similar to the function of mitochondria to provide energy for body activities. Mitophagy maintains cell homeostasis mainly through clearing damaged mitochondria and its expression is closely related to the activity of DRP1. As the source of Qi and blood biochemistry, the spleen is extremely important for maintaining the power required by the body. Related studies have found that strengthening the spleen increases the expression of Parkin and promotes mitophagy. Osteoporosis is one of the common complications of diabetes. Its onset is closely related to the decrease of muscle content caused by insulin resistance and microangiopathy, atherosclerosis caused by disorder of lipid metabolism and mitochondria degenerate due to oxidative stress induced by excessive production of oxidative free radicals in high glucose environment. Mitophagy eliminates the damaged mitochondria and reduces the damage to the mitochondria with sound structure and function to reduce the degree of insulin resistance and the risk of microangiopathy and atherosclerosis to prevent and treat diabetic osteoporosis. Therefore, based on the spleen - mitochondria theory, this paper discusses the relationship between mitophagy and diabetic osteoporosis.
