      目的 探讨脂联素对外周循环RANKL/OPG的调节作用,及对糖尿病性骨质疏松的治疗作用。方法 使用高脂饮食联合链脲佐霉素构建糖尿病性骨质疏松模型小鼠,使用脂联素治疗,使用阿仑膦酸钠治疗作为阳性对照。分组为:对照组、模型组、治疗组、阳性对照组。完成干预后,测量小鼠体重,收集小鼠静脉血并检测空腹血糖水平、葡萄糖耐量试验、血清RANKL/OPG。分别从各组小鼠股骨提取原代骨髓间充质干细胞,诱导成骨分化并茜素红染色,检测Opg、Rankl基因的表达量。获得小鼠腰3椎体,检测各组小鼠骨微细结构变化。结果 脂联素治疗后,与模型组相比,治疗组体重明显下降,空腹血糖水平明显下降,葡萄糖耐量试验明显接近正常水平,血清OPG水平上升,血清RANKL水平下降,血清RANKL/OPG下降。与对照组相比,模型组骨髓间充质干细胞成骨能力明显减弱,显著上调RANKL/OPG;与模型组相比,治疗组骨髓间充质干细胞成骨分化能力明显增强,显著下调RANKL/OPG。脂联素治疗后能显著提高单位长度成骨细胞数量、骨小梁面积百分数、骨小梁厚度。结论 脂联素能够降低糖尿病小鼠体重,降低空腹血糖,部分恢复机体的血糖调节能力,促进Opg基因的表达,抑制Rankl基因的表达,促进骨髓间充质干细胞成骨分化,改善骨微细结构,是一种潜在的治疗糖尿病性骨质疏松的选择。

      Objective To investigate the regulatory effect of adiponectin on RANKL/OPG in peripheral circulation and its therapeutic effect on diabetic osteoporosis. Methods High fat diet combined with streptozomycin was used to construct diabetic osteoporosis model mice, which were treated with adiponectin and alendronate sodium as positive control. They were divided into control group, model group, treatment group and positive control group. After the intervention, the weight of the mice was measured, the venous blood of the mice was collected, and the fasting glucose level, glucose tolerance test, and serum RANKL/OPG were detected. The primary bone marrow mesenchymal stem cells were extracted from the femur of mice in each group, induced osteogenic differentiation and stained with alizarin red to detect the expression levels of Opg and Rankl genes. The lumbar 3 vertebrae of mice were obtained, and the changes of bone microstructure in each group were detected. Results After adiponectin treatment, compared with the model group, the body weight, fasting blood glucose level and glucose tolerance test were significantly decreased, serum OPG concentration increased, serum RANKL concentration decreased, and serum RANKL/OPG decreased in the treatment group. Compared with the control group, the bone formation ability of bone marrow mesenchymal stem cells in the model group was significantly weakened, and RANKL/OPG was significantly up-regulated. Compared with the model group, the osteoblastic differentiation ability of bone marrow mesenchymal stem cells in the treatment group was significantly enhanced, and RANKL/OPG was significantly down-regulated. After adiponectin treatment, the number of osteoblasts per unit length, the percentage of trabecular area and the thickness of trabecular were significantly increased. Conclusion Adiponectin can reduce the body weight of diabetic mice, reduce fasting blood glucose, partially restore the body's blood glucose regulation ability, up-regulate Opg gene expression and inhibit Rankl gene expression, promote bone marrow mesenchymal stem cell osteogenic differentiation, and improve bone microstructure, which is a potential choice for the treatment of diabetic osteoporosis.
