      目的 比较双能X线吸收检测仪(dual-energy X-ray absorptiometry,DXA)检测中轴骨密度和外周骨密度的骨质疏松诊断结果,并研究DXA测量不同部位的骨密度及其随年龄变化的关系。方法 收集2020年12月至2022年2月在徐州医科大学附属医院骨质疏松科就诊的518例患者资料,年龄50~89岁,平均(67.8±8.8)岁。依据年龄分为4组,用DXA测量受试者中轴骨密度T值及外周骨密度T值,比较两者在诊断骨质疏松时的差异,并行一致性检验。记录各组腰1-4、全髋、股骨颈DXA T值,并观察各部位骨密度随年龄变化的特点。结果 中轴DXA和外周DXA诊断骨质疏松时差异有统计学意义(χ2=188.65,P<0.05),两种检查方法的一致性较弱(Kappa=0.2,P<0.05)。中轴骨密度各部位T值存在差异,且随年龄变化趋势不同。结论 中轴骨密度结合外周骨密度可以提高骨质疏松的检出率,中轴骨不同部位间骨密度 T值不能相互替代。

      Objective To compare the results of dual-energy X-ray absorptiometry (DXA) for the diagnosis of osteoporosis by the detection of mid-axial and peripheral bone density, and to study the relationship between DXA measurements of bone density at different sites and its changes with age. Methods 518 patients aged 50-89 with an average age of (67.8 ± 8.8) years were collected from the Osteoporosis Department of the Affiliated Hospital of Xuzhou Medical University from December 2020 to February 2022. Divided the subjects into 4 groups based on age, measured the T-scores of axial bone density and peripheral bone density using DXA, compared the differences between the two in diagnosing osteoporosis, and conducted consistency tests. Recorded the DXA T-scores of lumbar 1-4, total hip, and femoral neck in each group, and observed the characteristics of bone density changes in each area with age. Results The difference between mid-axial DXA and peripheral DXA in the diagnosis of osteoporosis was statistically significant (χ2= 188.65, P < 0.05), and the concordance between the two tests was statistically significant, with a weak concordance (Kappa = 0.2, P < 0.05). There were differences in T-scores between sites of mid-axial BMD and different trends with age. Conclusion The combination of mid-axial BMD with peripheral BMD can improve the detection rate of osteoporosis, and BMD T-scores between different parts of the mid-axial bone cannot be substituted for each other.
