      “骨极”理论来源于中医学“五劳六极七伤”学说,是祖国医学对虚劳后期病机和症状的高度概括,病机重点在“虚劳感邪,邪久不解,内客肾脏,发为骨极”,临床表现为“手足烦疼,不可以立,不欲行动”。骨质疏松症是一种典型的骨代谢性疾病,其发病与骨重塑过程被破坏密切相关,而骨形成和骨吸收间的动态平衡是维持正常骨重塑功能的基础。“骨极”理论描述的病机和症状与骨质疏松症高度相似,是阐释骨质疏松症发病机理的重要切入点。细胞自噬是细胞分化和增殖的重要过程。越来越多的证据表明,自噬对成骨细胞、破骨细胞及骨髓间充质干细胞都具有重要的调节作用。自噬水平异常导致的骨稳态失衡是骨质疏松症发病的关键因素。基于论证,笔者提出了一种新兴的“骨极-细胞自噬-骨质疏松症”学说,以期从“骨极”理论切入,探讨自噬与骨质疏松症的关系,为骨质疏松症的预防和治疗以及进一步的研究提供新的理论参考依据。

      The theory of bone-pole is derived from the doctrine of five-fatigue, six-pole, and seven-damage. Bone-pole is a high-level generalization of the pathological changes and clinical symptoms of consumptive disease in Traditional Chinese Medicine. Bone-pole is developed from consumptive disease in Traditional Chinese Medicine. The clinical manifestations of bone-pole are pain in the hands and feet and inability to stand and walk for long period of time. Osteoporosis is a typical bone metabolic disease. Its pathogenesis is closely related to the disruption of the bone remodeling process. The balance between bone formation and bone resorption is a basis for maintaining normal bone remodeling function. The content of bone-pole is similar to the manifestation of osteoporosis, which is an important entry point for elucidating the pathogenesis of osteoporosis. Autophagy is an important process in cell differentiation and proliferation. Increasing evidence suggests that autophagy has an important regulatory role in osteoblasts, osteoclasts, and bone marrow mesenchymal stem cells. Imbalance of bone homeostasis due to abnormal autophagy levels is a key factor in the pathogenesis of osteoporosis. Therefore, this article discusses the progress of research in the prevention and treatment of osteoporosis from the perspective of bone-pole and autophagy.
