      骨质疏松症是一种常见的骨骼系统疾病,其主要特征是骨密度减少和骨质结构疏松,易于发生骨折。骨稳态的平衡维持了骨骼的结构和功能。当骨吸收过度或骨形成不足时,骨稳态被打破,导致骨质疏松症的发生。核因子-κB(NF-κB)受体活化因子配体(RANKL)信号通路在骨稳态调节、骨质疏松症发生发展过程中扮演着重要的角色。RANKL是一种细胞因子,通过与其受体核因子-κB受体激活因子(RANK)结合,促进破骨细胞的形成和活化,导致骨质疏松症的发生。中药作为中医治疗体系中的核心组成部分,在防治骨质疏松症方面有着悠久的历史、丰富的实践经验和深远的潜力。针对中药干预RANKL信号通路调控破骨细胞治疗骨质疏松症的作用机制,近年来取得了一些研究进展。研究表明,中药可通过抑制RANKL的表达和活性、降低破骨细胞的形成和活化、促进骨形成、调节骨吸收和骨形成平衡、抗炎和抗氧化等,发挥治疗骨质疏松症的作用。同时,中药具有整体调节、副作用较少、患者接受度和满意度较高等优势,在防治骨质疏松症方面占据重要地位。因此,笔者系统梳理了最新中药干预RANKL信号通路调控破骨细胞治疗骨质疏松症的相关文献,旨在临床防治骨质疏松症的中药新药研发提供一定依据。

      Osteoporosis is a common disease of the skeletal system,which is characterized by reduced bone mineral density and osteoporosis,and is prone to fracture. The balance of bone homeostasis maintains the structure and function of bone. When bone resorption is excessive or bone formation is insufficient,bone homeostasis is broken,leading to the occurrence of osteoporosis. The nuclear factor-κB (NF-κB) receptor activator ligand (RANKL) signaling pathway plays an important role in the regulation of bone homeostasis and the development of osteoporosis. RANKL is a cytokine that promotes the formation and activation of osteoclasts by binding to its receptor nuclear factor-κB receptor activator (RANK),leading to the occurrence of osteoporosis. As a core component of the traditional Chinese medicine treatment system,traditional Chinese medicine has a long history,rich practical experience and far-reaching potential in the prevention and treatment of osteoporosis. In recent years,some research progress has been made on the mechanism of Chinese medicine intervention in RANKL signaling pathway to regulate osteoclasts in the treatment of osteoporosis. Studies have shown that traditional Chinese medicine can play a role in the treatment of osteoporosis by inhibiting the expression and activity of RANKL,reducing the formation and activation of osteoclasts,promoting bone formation, regulating bone resorption and bone formation balance,anti-inflammatory and anti-oxidation. At the same time,traditional Chinese medicine has the advantages of overall regulation,less side effects,higher patient acceptance and satisfaction,and plays an important role in the prevention and treatment of osteoporosis. This article systematically reviews the latest literature on the intervention of RANKL signaling pathway by traditional Chinese medicine to regulate osteoclasts in the treatment of osteoporosis,aiming to provide a basis for the development of new Chinese medicine for clinical prevention and treatment of osteoporosis.
