      膝骨关节炎(knee osteoarthritis,KOA)是临床常见的退行性骨关节疾病,软骨破坏是其关键病理标志,修复软骨损伤被认为是治疗KOA最具有前景的方法之一。目前针对KOA的治疗如药物、关节镜下清创术等主要是对症治疗和疼痛管理。研究发现干细胞具有自我更新和定向分化潜能,可修复软骨组织并抑制软骨细胞分泌炎症因子等,临床试验也证实干细胞治疗KOA软骨损伤有显著效果,但有关干细胞成软骨细胞分化以及软骨再生相关机制缺少系统的阐述。笔者从干细胞成软骨细胞分化、调节细胞外基质和炎症微环境等方面,针对干细胞修复软骨损伤治疗KOA的机制及临床应用作一综述,以期为治疗KOA及组织再生领域提供一定的理论和思路。

      Knee osteoarthritis (KOA) is a common degenerative osteoarthritic disease. Cartilage destruction is the key pathological characteristic of KOA,and repairing it is considered one of the most promising approaches for treating KOA. Current treatments for KOA,such as drugs and arthroscopic debridement,are mainly symptom control and pain management. Stem cells have the potential of self-renewal and directional differentiation,and they can repair cartilage tissues and inhibit the secretion of inflammatory factors by chondrocytes. Clinical trials have also verified the significant effects of stem cells on cartilage injury in KOA. However,there is a lack of systematic elaboration on the mechanisms of differentiation of stem cells into chondrocytes and cartilage regeneration. In this paper,we review the mechanisms and clinical applications of the repair of cartilage injury by stem cells in the treatment of KOA, from the aspects of differentiation of stem cells into chondrocytes and regulation of extracellular matrix and inflammatory microenvironment. We aim to provide theories and ideas for KOA treatment and tissue regeneration.
