| 目的 探讨患者血清中NLR、PTX3及14-3-3η蛋白对类风湿关节炎患者并发骨质疏松的预测价值。方法 回顾性分析2019年1月至2021年8月九江市第一人民医院收治的130例类风湿关节炎患者，根据其是否并发骨质疏松将其分为并发组及非并发组，其中并发组43例（33.08%），非并发组87例（66.92%），收集两组患者相关资料，对其进行危险因素分析并构建风险列线图模型，另选取2021年9月至2022年12月收治的55例类风湿关节炎患者作为验证组，收集其相关资料，对风险列线图模型进行验证。结果 对类风湿关节炎患者并发骨质疏松的因素分析显示IL-17、CRP、NLR、PTX3及 14-3-3η蛋白是独立危险因素，Logit(P)=0.181×IL-17+0.174×CRP+2.890×NLR+3.406×PTX3+0.632×14-3-3η蛋白-50.616。其ROC曲线下面积为0.907，95% CI（0.975~1.000），灵敏度为93.0%，特异度为97.7%。结论 血清中NLR、PTX3及14-3-3η蛋白等指标在并发或不并发骨质疏松的类风湿关节炎患者中具有显著差异，其中IL-17、CRP、NLR、PTX3及14-3-3η蛋白联合预测时具备更高的预测价值。 |

| Objective To investigate the predictive value of serum NLR，PTX3 and 14-3-3-η proteins in patients with rheumatoid arthritis complicated with osteoporosis. Methods A retrospective analysis was performed on 130 patients with rheumatoid arthritis admitted to our hospital from January 2019 to August 2021，who were divided into a concurrent group and a non-concurrent group according to whether they had osteoporosis，including 43 cases (33.08%) in the concurrent group and 87 cases (66.92%) in the non-concurrent group. Relevant data of the two groups of patients were collected. Risk factors were analyzed and a risk nomogram model was constructed. In addition，55 patients with rheumatoid arthritis admitted to our hospital from September 2021 to December 2022 were selected as the verification group，and relevant data were collected to verify the risk nomogram model. Results IL-17，CRP，NLR，PTX3 and 14-3-3-η protein were independent risk factors of osteoporosis in patients with rheumatoid arthritis.
Logit(P)=0.181×IL-17+0.174×CRP+2.890×NLR+3.406×PTX3+0.632×14-3-3ηprotein-50.616. The area under ROC curve was 0.907，95%CI (0.975-1.000)，sensitivity was 93.0%，specificity was 97.7%. Conclusion Serum NLR, PTX3 and 14-3-3η protein are significantly different in rheumatoid arthritis patients with or without osteoporosis，and IL-17，CRP，NLR，PTX3 and 14-3-3η protein have higher predictive value when combined. |
