      目的 探讨中老年人群基线抑郁症状与骨质疏松症及骨折发病风险的相关性。方法 选取2011年开展的“REACTION”研究中40~75岁兰州地区居民,并于2014年至2016年进行随访,共纳入3 615例调查资料完整的对象进行研究。使用患者健康问卷(PHQ-9)对抑郁症状进行评估和分组,比较不同组别骨质疏松症及骨折的发病情况,采用Cox比例风险模型研究抑郁症状及其严重程度对骨质疏松症及骨折发病的影响,采用多因素线性回归分析抑郁症状评分与跟骨超声参数的相关性。结果 ①平均随访3.4±0.5年后,新发骨质疏松症1 228人,新发骨折138人;②Cox回归分析显示,抑郁症状与骨质疏松症及骨折发病风险无关(P>0.05),抑郁症状按严重程度分级后,也未观察到相关性(P>0.05);③多因素线性回归结果显示,抑郁症状评分与跟骨超声参数呈负相关(P<0.05)。结论 抑郁症状评分越高,定量超声参数越低,本研究未发现基线抑郁症状与骨质疏松症及骨折的发病风险有关。

      Objective To investigate the correlation between baseline depressive symptoms and the risk of osteoporosis and fracture in middle-aged and elderly population. Methods Residents aged 40-75 years in Lanzhou from the “REACTION ”study conducted in 2011 were selected and followed up from 2014 to 2016. A total of 3615 subjects with complete data were analyzed. Patient Health Questionnaire (PHQ-9) was used to evaluate and group the depressive symptoms, and the incidence of osteoporosis and fracture in different groups were compared. Cox proportional hazards model was used to explore the impact of depressive symptoms and their severity on the incidence of osteoporosis and fracture. Multivariate linear regression was used to analyze the correlation between depressive symptom scores and calcaneal ultrasound parameters. Results ①After a mean follow-up of 3.4±0.5 years, there were 1228 new cases of osteoporosis and 138 new cases of fracture. ② Cox regression analysis showed that depressive symptoms were not associated with the risk of osteoporosis and fracture (P>0.05), and no correlation was observed after grading depressive symptoms according to severity (P>0.05); ③ Multivariate linear regression results showed that depressive symptom score was negatively correlated with calcaneal ultrasound parameters (P<0.05). Conclusion The higher the depressive symptom score, the lower the quantitative ultrasound parameters, There is no evidence that baseline depressive symptoms are associated with the risk of osteoporosis and fracture.
