| Diabetic osteoporosis (DOP), as a severe complication of diabetes, is not only closely related to the reduction of bone mass but also significantly increases the risk of fractures. Its pathogenesis is complex and involves various factors, including the loss of osteocyte vitality, hyperglycemia, and insulin resistance. From the multi-dimensional perspective of traditional Chinese medicine (TCM), the treatment of DOP can be divided into three aspects: treatment from the perspective of the five viscera, treatment from the perspective of collateral disease and blood stasis, and treatment from the perspective of biological axis interaction. In the treatment from the perspective of the five viscera, the emphasis is on nourishing the kidneys and spleen, and supplementing the liver and kidneys. In the treatment from the perspective of collateral disease and blood stasis, methods for regulating blood vessels and promoting bone formation are proposed. In the treatment from the perspective of biological axis interaction, the impact of the kidney-brain-bone axis, spleen-intestine axis, and inflammation-metabolism axis on DOP is discussed. Although traditional Chinese medicine has shown certain efficacy in the treatment of DOP, further improvement of evidence-based basis, strengthening of prevention strategies, precise evaluation with modern technology, promotion of integrated traditional Chinese and Western medicine and standardization research, and promotion of the scientific and modern process of DOP prevention and treatment research are still needed. |
