      目的 探讨补肾健脾活血方对绝经后骨质疏松症(postmenopausal osteoporosis,PMOP)模型大鼠骨骼肌相关蛋白因子以及GPX4表达情况的影响。方法 选取24只3月龄SPF级雌性大鼠,使用随机数字表将大鼠分为假手术组(n=6)和模型组(n=18),模型组又分为模型对照组、补肾健脾活血方组和阿仑膦酸钠组,每组各6只,灌胃干预12周后取材。取材后使用双能X射线吸收法测定大鼠骨密度及体成分;MicroCT检测股骨远端骨微结构;比目鱼肌HE染色及肌纤维横截面积测定;蛋白免疫印迹法检测大鼠肌肉组织中肌肉生成分化因子(myogenic differentiation,MYOD)和肌肉调节因子5(myogenic factor 5,MYF5)蛋白的表达量;免疫组化观察大鼠肌肉组织中谷胱甘肽过氧化物酶4(glutathione peroxidase 4,GPX4)的变化。结果 与假手术组相比,模型组大鼠骨密度(bone mineral density,BMD)降低(P<0.0001),脂肪含量显著提高;骨小梁数量减少,骨微结构破坏;比目鱼肌横截面积缩小;MYOD和MYF5的蛋白表达量下降;肌肉组织中GPX4表达减少。与模型组对比,补肾健脾活血方组大鼠骨密度升高,骨微结构、肌纤维横截面积改善,MYOD、MYF5和GPX4表达上升。结论 补肾健脾活血方在改善PMOP大鼠骨量丢失的同时,也能通过提高肌肉调控因子MYOD和MYF5水平防治PMOP造成的骨骼肌萎缩。肌肉铁过载可能与PMOP相关肌少症的发生发展有关。

      Objective To explore the effects of the formula of tonifying kidney, strengthening spleen, and activating blood on the skeletal muscle and its related protein factors in rat postmenopausal osteoporosis (PMOP) model. Methods Twenty-four 3-month-old SPF-grade female rats were selected and divided into a sham-operation group (n=6) and a model group (n=18) using a table of random numbers. The model group was further divided into a model control group, a group of kidney and spleen strengthening and blood activating formula group, and a group of alendronate, with 6 rats in each group. The samples were collected after 12 weeks of gavage intervention. Bone mineral density (BMD) and body composition of the rats were determined using dual-energy X-ray absorptiometry. Bone microstructure of the distal femur was detected using micro CT. HE staining of the flounder muscle and determination of the cross-sectional area of muscle fibers were performed. Myogenic differentiation (MYOD) and myogenic factor 5 (MYF5) were detected with Western blotting in the muscle tissues of rats. The changes of glutathione peroxidase 4 (GPX4) in rat muscle tissues were observed with immunohistochemistry. Results Compared to those in the sham-operated group, rats in the model group showed lower BMD (P<0.0001) and significantly higher fat content, reduced number of bone trabeculae and destruction of bone microstructure, reduced cross-sectional area of the soleus muscle, decreased protein expressions of MYOD and MYF5, and reduced expression of GPX4 in muscle tissue. Compared to those in the model group, rats in the tonifying kidney, strengthening spleen, and activating blood formula group showed increased BMD, improved bone microstructure and cross-sectional area of muscle fibers, and increased expressions of MYOD, MYF5 and GPX4. Conclusion While preventing bone loss in PMOP rats, tonifying kidney, strengthening spleen, and activating blood formula also prevents and controls PMOP-induced skeletal muscle atrophy by increasing the levels of muscle regulatory factors MYOD and MYF5. Muscle iron overload may be related to the development of PMOP-associated sarcopenia.
