血钙、血磷浓度及血清25(OH)D对绝经后OP辅助诊断的临床价值

      目的 探究血清中钙和磷的浓度及25-羟基维生素D[25(OH)D]在绝经后骨质疏松症(osteoporosis,OP)患者的临床诊断价值。方法 选取2022年1月至2023年1月于我院就诊的103例绝经后OP患者(观察组)和期间我院收治的100名非OP体检者(对照组)为研究对象。对比两组参与者的骨密度(腰椎、左股骨颈、右股骨颈),对比参与者的血钙、血磷和血清25(OH)D水平,采用ROC曲线评估3种生化指标单一和联合检测效能;采用Pearson相关性分析生化指标和骨密度的相关性。结果 观察组的骨密度水平低于对照组(P<0.05);观察组的血钙和血清25(OH)D水平低于对照组(P<0.05);两组患者的血磷对比无统计学意义(P>0.05)。血钙和血清25(OH)D水平联合检测(95.31%、92.35%、0.838、0.929)的灵敏度、特异度、约登指数、AUC面积低于血钙(88.15%、83.45%、0.754、0.823)和血清25(OH)D水平(79.00%、85.12%、0.679、0.756)单一检测(P<0.05);血钙、血清25(OH)D和骨密度呈正相关(r>1,P<0.05);血磷和骨密度无相关性(P>0.05)。结论 OP患者血钙、血清25(OH)D水平较正常人变化较为明显,可用于绝经后OP辅助诊断中。

      Objective To explore the clinical diagnostic value of serum calcium, phosphorus, and 25-hydroxyvitamin D [25 (OH) D] concentrations in menopausal women with osteoporosis (OP). Methods One hundred and three OP patients treated in our hospital from January 2022 to January 2023 (observation group) and 100 non-OP physical examination patients treated in our hospital during the period (control group) were selected as the study objects. Bone mineral density (the lumbar spine, left femoral neck, and right femoral neck), blood calcium, blood phosphorus, and serum 25 (OH) D levels were compared between the two groups. ROC curve was used to evaluate the efficacy of single and combined detection of the three biochemical indicators. Pearson correlation was used to analyze the correlation between biochemical indexes and bone mineral density. Results Bone mineral density in the observation group was higher than that in the control group (P<0.05). The serum calcium and serum 25 (OH) D levels in the observation group were lower than those in the control group (P<0.05). There was no significant difference in blood phosphorus between the two groups (P>0.05). The sensitivity, specificity, Jordon index, and AUC area of serum calcium and serum 25 (OH) D level (95.31%, 92.35%, 0.838, 0.929) were lower than those of serum calcium (88.15, 83.45, 0.754, 0.823) and serum 25 (OH) D level (79.00, 85.12, 0). 679, 0.756) single detection (P<0.05). Serum calcium and serum 25 (OH) D were positively correlated with BMD (r>1, P<0.05). There was no correlation between blood phosphorus and bone mineral density (P>0.05). Conclusion The changes of serum calcium and 25 (OH) D in OP patients are more obvious than those in normal subjects, which can be used in the auxiliary diagnosis of OP in postmenopausal women.
