中药调控NF-κB通路治疗类风湿性关节炎研究进展

      类风湿性关节炎(RA)是一种影响全身关节的慢性、进展性疾病,其发病机制不明,致死致残率高,且目前尚无根治性治疗手段。核转录因子-κB(NF-κB)通路是防治RA的重要途径,可有效抑制滑膜炎症、调节免疫、减少骨破坏、促进软骨修复。中医药治疗RA具有明显优势,可有效延缓疾病进展且不良反应少,而NF-κB通路是中医药治疗RA的关键途径。现代研究显示中医药可通过调控NF-κB通路,发挥影响巨噬细胞极化、滑膜成纤维细胞(FLS)增殖与表型转化、破骨细胞(OCs)分化和增殖以及软骨细胞凋亡与存活的作用。基于此,文章阐述了NF-κB通路在RA发病机制中的关键作用,并总结归纳近5年来中药单体如苷类、萜类、黄酮类、生物碱类、酚类以及经典方剂、名医验方、中成药等通过调控NF-κB通路治疗RA的研究成果,以期为临床应用中药防治RA和推动中药新药开发提供参考。

      Rheumatoid arthritis (RA) is a chronic, progressive disease affecting the joints of the whole body, its pathogenesis is unknown, the death rate is high, and there is no radical treatment. Nuclear transcription factor-κB (NF-κB) pathway is an important pathway to prevent and cure RA, which can effectively inhibit synovial inflammation, regulate immunity, reduce bone destruction, and promote cartilage repair. Traditional Chinese medicine has obvious advantages in the treatment of RA, which can effectively delay the progression of the disease with less toxic side effects, and NF-κB pathway is the key pathway of traditional Chinese medicine in the treatment of RA. Modern studies have shown that Chinese medicine can affect the polarization of macrophages, proliferation and phenotypic transformation of synovial fibroblasts (FLS), differentiation and proliferation of osteoclasts (OCs), and apoptosis and survival of chondrocytes by regulating the NF-κB pathway. Based on this, this paper describes the key role of NF-κB pathway in the pathogenesis of RA, and summarizes the research results of traditional Chinese medicine monomers such as glycosides, terpenoids, flavonoids, alkaloids, phenols, classical prescriptions, famous medical prescriptions, and proprietary Chinese medicines in the treatment of RA by regulating NF-κB pathway in the past five years. In order to provide reference for clinical application of traditional Chinese medicine to prevent RA and promote the development of new traditional Chinese medicine.
