      目的 探究富血小板血浆复合振动治疗对骨质疏松性骨折(osteoporotic fracture,OPF)大鼠的治疗影响。方法 收集SPF级大鼠40只,随机分为对照组、模型组、富血小板血浆组(PRP)、联合组,10只/组。除对照组外其余均建立OPF模型,建模后对照组及模型组不进行处理,PRP组于骨折处注入PRP,联合组在PRP组基础上行振动治疗,均干预30 d。采用计算机X射线观察骨愈合,生物力学试验机检测骨载荷变形曲线、最大载荷和弹性模量,HE染色观察病理变化,Scanco μCT-35仪器检测骨小梁相关参数,免疫印迹法检测RANKL、骨保护蛋白。结果 与对照组比较,模型组骨折弹性模量、最大载荷、断裂点载荷、皮质骨面积、骨小梁厚度、骨小梁面积、骨小梁体积、骨保护蛋白降低,RANKL升高(P<0.05);与模型组比较,PRP治疗组骨折愈合评分、弹性模量、最大载荷、断裂点载荷、皮质骨面积、骨小梁厚度、骨小梁面积、骨小梁体积、骨保护蛋白升高,RANKL降低(P<0.05);与PRP治疗组相比,联合组骨折愈合评分、弹性模量、最大载荷、断裂点载荷、骨小梁厚度、皮质骨面积、骨小梁体积、骨小梁面积、骨保护蛋白升高,RANKL降低(P<0.05)。结论 富血小板血浆与振动联合治疗可改善OPF大鼠的疾病状态,其机制与改善骨愈合、生物力学及骨组织形态相关。

      Objective To explore the therapeutic effect of platelet-rich plasma combined with vibration therapy on OPF rats. Methods Forty SPF rats were collected and randomly divided into control group, model group, platelet-rich plasma group (PRP) and combination group (10 rats/group). All OPF models were established except the control group. After modeling, the control group and model group were not treated, PRP was injected into the fracture site in the PRP group, and the combined group was treated with basic upward vibration therapy in the PRP group, with intervention for 30 days. Bone healing was observed by computer X-ray, bone load deformation curve, maximum load and elastic modulus were detected by biomechanical testing machine, pathological changes were observed by HE staining, bone trabecular related parameters were detected by Scanco μCT-35 instrument, and RANKL and bone protective protein were detected by western blot. Results Compared with the control group, the fracture elastic modulus, maximum load, breaking point load, cortical bone area, trabecular thickness, trabecular area, trabecular volume, bone protective protein and RANKL were decreased and increased in the model group (P<0.05). Compared with model group, fracture healing score, elastic modulus, maximum load, breaking point load, cortical bone area, trabecular thickness, trabecular area, trabecular volume, bone protective protein and RANKL were increased and decreased in PRP treatment group (P<0.05). Compared with PRP treatment group, fracture healing score, elastic modulus, maximum load, breaking point load, trabecular thickness, cortical bone area, trabecular volume, trabecular area, bone protective protein and RANKL were increased and decreased in combination group (P<0.05). Conclusion Combined treatment with platelet-rich plasma and vibration can improve the disease status of osteoporotic fracture rats, and its mechanism may be related to improved bone healing, biomechanics and bone morphology.
