| 目的 探讨老年2型糖尿病(type 2 diabetes mellitus,T2DM)患者血清钙磷、白介素-1β(IL-1β)、25羟基维生素D[25(OH)D]表达与骨质疏松症(osteoporosis, OP)发病的相关性。方法 将在新乡市中心医院就诊的200例老年T2DM患者,按是否合并OP进行分组,将130例单纯T2DM患者作为对照组,70例T2DM伴OP者归入研究组,比较两组血清钙磷、IL-1β、25(OH)D水平,并以Pearson相关系数分析IL-1β、钙磷、25(OH)D与OP的关联性。结果 研究组血钙、血磷、25(OH)D水平均低于对照组,IL-1β水平高于对照组,P<0.05;研究组骨钙素水平低于对照组,甲状旁腺素、β-胶原特殊序列高于对照组,P<0.05;研究组股骨颈骨密度、腰椎L1~4骨密度、全髋骨密度、骨密度均值低于对照组,P<0.05。Pearson相关分析结果显示,血清钙磷、25(OH)D表达与β-胶原特殊序列、甲状旁腺素水平呈反比,与骨钙素及骨密度指标均呈正比。而IL-1β水平与骨代谢指标(甲状旁腺素水平、β-胶原特殊序列)呈正相关,IL-1β表达与骨钙素、骨密度水平呈负相关。结论 老年T2DM患者25(OH)D、血清钙磷水平降低,IL-1β高表达与患者骨密度、骨代谢存在显著关联性,与机体糖尿病性骨质疏松症的发生、发展关系密切。 |

| Objective To investigate the correlation between the expression of serum calcium and phosphorus, interleukin-1β (IL-1β), 25 hydroxyvitamin D (25(OH)D) and the incidence of osteoporosis (OP) in elderly patients with type 2 diabetes mellitus (T2DM). Methods A total of 200 elderly T2DM patients were divided into two groups according to whether they had OP or not. 130 T2DM patients were divided into control group and 70 T2DM patients with OP were divided into study group. Serum levels of calcium and phosphorus, IL-1β and 25(OH)D were compared between the two groups. The correlations between IL-1β, Ca and P, 25(OH)D and OP were analyzed by Pearson correlation coefficient. Results The levels of blood calcium, blood phosphorus, and 25(OH)D in the study group were lower than those in the control group, and level of IL-1β was higher than that in the control group(P<0.05); the level of bone calcitonin in the study group was than that in the control group, and the levels of parathyroid hormone and β-special sequence collagen were higher than those in the control group(P<0.05); The bone mineral density of femoral neck, lumbar spine L1-4, total hip, and mean bone mineral density in the study group were lower than those the control group(P<0.05). Pearson correlation analysis showed that serum calcium, phosphorus and 25(OH)D expression were inversely proportional to β-collagen sequence and parathyroid hormone levels, and positively correlated with osteocalcin and bone mineral density. The level of IL-1β was positively correlated with bone metabolism indexes (parathyroid hormone level, β-collagen special sequence), and the expression of IL-1β was negatively correlated with osteocalcin and bone mineral density. Conclusion 25(OH)D, decreased serum calcium and phosphorus levels, and high expression of IL-1β in elderly patients with T2DM are significantly correlated with bone mineral density and bone metabolism, and are closely related to the occurrence and development of diabetic osteoporosis. |
